Equipment Failure Mid-Shift Response
Purpose
A machine that dies at the second stop is a decision problem before it is a repair problem: the crew is in somebody's home, the clock is running, and the cheapest-looking move is to nurse the machine through the day. This standard guarantees three things: the machine is isolated before anyone diagnoses it, it cannot be run by the next person who opens the van, and the route is re-scoped by whoever owns that call, with the customer told before the crew leaves.
Scope
Covers cord-and-plug and battery cleaning equipment on a residential or small-commercial route - uprights, backpack vacuums, extractors, swing machines, cordless tools - from the moment behavior changes to the handoff at the shop bench.
Does not cover scheduled service (vacuum and HEPA filter maintenance standard) or work the customer ADDS mid-visit (adding-a-task standard); this is the mirror case, work a machine SUBTRACTS. No cord, switch, motor, pump or heater repair happens on a route, and a customer's own equipment is never worked on.

A hot, swollen, hissing or sharp-smelling pack is not class 2. It takes the step 1 path and is never swapped or charged.

Procedure
Step 1 - Isolate at the machine's own isolation point before you look at anything. Switch off, pull the plug and keep it in your hand; on a battery machine, pull the pack. Under 29 CFR 1910.147(a)(2)(iii) that is isolation only while the plug stays in your own control. Acceptance: plug out and in your sight, or pack out and clear. Wrong: switched off but still plugged in. Stop: smoke, burnt insulation, hot rubber, a hot plug or a tripped GFCI means no restart. Hazard: a shorted motor can restart into an arc, so the plug comes out; a hot, swollen or hissing pack goes outside onto a non-combustible surface clear of the building, never onto a charger or into the van; fire service if it vents.
Step 2 - Clear the hazard the failure created in the customer's space. Free water, a cord across a doorway, a spilled tank, a machine in a stair path. Acceptance: no free water on a walking surface without a barrier or sign, cord coiled clear, machine out of the egress path, children and pets clear. Wrong: leaving it in the hall while you go to the van. Stop: a release bigger than a wipe-up, or one nobody can identify, ends work in that room and the SDS routes disposal. Hazard: spilled concentrate is a contact route, so SDS section 8 gloves and eye protection before a hand touches it, and skin or eye contact is flushed for the time the label states.
Step 3 - Name the failure against the boundary table before opening anything. One pass, still isolated: what changed, when, and which of the six classes it is. Acceptance: the symptom in the operator's words plus one class number, written down. Wrong: a teardown on the customer's floor on a hunch about the belt. Stop: classes 3 through 6 go to step 6 with no repair attempted, and a cord defect keeps it out of use until repaired. Hazard: none while it stays isolated; a hand inside a housing means step 4.
Step 4 - Do the field fix at the van, out of the occupied space, machine still isolated. Class 1 and class 2 only, as the table lists them. Acceptance: plug in sight or pack out throughout, old bag sealed and double-bagged at the machine, no tool on an electrical or pressure-bearing part. Wrong: knocking a canister out over the customer's bin. Stop: a fix needing a part you do not carry stops here, and so does a second fix of the same kind on that machine the same shift, because two of one fix in a day is a bearing or roll-height problem. Hazard: opening it reopens the inhalation route, so work outdoors or at the station with local exhaust, shake nothing, no compressed air on a bag or filter, and a respirator only if the wearer is in a written program under 29 CFR 1910.134; if nobody on shift is, the crew does not do that work. If it ran on drywall, masonry or pre-1978 painted-surface dust the bag stays shut. Wrap holds needles and glass, so cut-resistant gloves on, and a sharp that breaks skin is a bloodborne exposure under 29 CFR 1910.1030 started that day.
Step 5 - Prove it at the van before it re-enters the home, protective function included. Acceptance: switch off before the plug seats; 60 seconds running under load, a shop default to tune; no burning smell and no plume at the exhaust; no weep from a water-bearing joint held 2 minutes under working pressure, also a shop default; and the protective function proved - brushroll shutoff stopping the roll with the handle upright, dead-man releasing the disc, GFCI tripping on its test button and resetting. Wrong: a machine judged fixed because it sounds right. Stop: any one failing sends it to step 6 even with the original fault gone, because a fresh belt behind a dead shutoff is worse than the broken belt was. Hazard: this puts energy back into a machine you just had open, where the customer can walk up, so plug in with the switch off and your hand clear of the roll; a machine that starts as the plug seats has a failed switch and is tagged there.
Step 6 - Red-tag it so the next person cannot run it by accident. Acceptance: tag at the control or the plug rather than the handle, carrying six fields - machine ID, date and time, stop, symptom in the operator's words, what was attempted, who tagged it - cord wrapped or pack bagged separately, machine kept apart from ready stock. Wrong: a dead machine at the back of the van, untagged. Stop: out of tag stock does not mean it travels bare; tape across the switch with the symptom on it is the floor. Hazard: a pack that failed hot stays outside per step 1 rather than riding in the van, and a machine that took up water is not run until the shop confirms it dry inside.
Step 7 - Re-scope the route with the office within 15 minutes of the tag, and tell the customer before you leave. Acceptance: office contacted inside 15 minutes; every task left today marked deliverable, substitutable or deferred; each substitution named as a METHOD, not another machine; the customer told what will not happen and what happens instead. Wrong: delivering most of the stop and letting the customer find the gap. Stop: a task whose only method is the dead machine is deferred, not faked - suction-only is not an agitation pass on cut pile - and routes to the complaint and redo standard as scheduled work with a date; a tagged machine is not re-run because the customer asks. Hazard: name the substitute route first: hand-scrubbing a floor puts a person on their knees inside the wet zone the wet floor standard governs.
Step 8 - Hand the machine and the tag to the shop the same day and leave it out of service. Acceptance: tag intact on delivery, equipment log updated with downtime in minutes and the symptom as written, tag removed only by the shop after the maintenance standard's return-to-service check. Wrong: a tag pulled by the crew that borrowed the machine back for one stop. Stop: the same machine ID tagged twice inside 30 days gets no third repair until the owner decides repair against replace; 30 days is a shop default to tune. Hazard: none, a handoff at a bench.
The record this produces
The tag carries the six fields step 6 names. The symptom in the operator's own words is the field most often lost and most useful later: "smelled hot, then the roll stopped and it still pulled air" tells a shop tech where to start, and "broken" does not.
The ticket carries minutes lost, tasks deferred by name, the substitute method and who told the customer. The log carries downtime by machine ID, the repair and the tag-clearing check, which makes the step 8 repair-against-replace call an evidence question.
Worked pass: an upright dies at the second stop of a four-stop day
Stop 2 is a 150 minute visit, two-person crew, so 300 labor minutes. Every figure below is clock time on the cleaner who owned the upright, against that 150 minute stop window; the second cleaner stayed on their own list throughout, so the response cost one person's minutes rather than two and the minutes delivered are this cleaner's lane alone. At minute 40 the upright gives a rubber smell, the roll stops turning, and it still pulls air.
Seven timed segments follow. Step 1, 2 minutes: off, unplugged, plug in hand. Step 2, 5 minutes: cord out of the doorway, machine to the laundry room, dogs out with the customer. Step 3, 3 minutes: roll free by hand, no burnt-insulation smell, no GFCI trip, so class 1, written as "belt snapped, smelled hot". Step 4, 12 minutes: belt from truck stock, roll stripped in cut-resistant gloves. Step 5, 4 minutes, of which 60 seconds running under load: no burning smell, no plume.
Step 5 then failed on its last criterion. With the handle upright the brushroll kept turning, so the stop rule ran and the machine was tagged with the belt fixed. Step 6, 3 minutes: tagged at the plug with all six fields, tag on at minute 69. Step 7, 6 minutes from minute 72: hard floors and low pile went to suction-only passes plus hand-worked edges, carried by the backpack vacuum, and two cut-pile bedrooms with heavy pet hair could not meet the standard without powered agitation, so they were deferred and booked as a redo.
The arithmetic: 2 + 5 + 3 + 12 + 4 + 3 + 6 is 35 minutes of response, plus 3 minutes waiting on the office, so work resumed 38 minutes after the failure, at minute 78. Of the 110 minutes still planned at minute 40 (150 budget minus 40 elapsed), the deferred bedrooms were 12 minutes and the hand-worked edges added 15, so 110 minus 12 plus 15 leaves 113 delivered. Total: 40 plus 38 plus 113 is 191 minutes on this cleaner's clock against the stop's 150 minute window, 41 minutes over, and the office moved stop 4 to next morning rather than start it that late.
Two figures need their basis said out loud. The 2 minute pressure hold did not apply, since an upright has no water-bearing part, so the governing criterion was the shutoff check, and it failed. The 12 and 15 minute figures are measured off this ticket, not thresholds from any step.
Exceptions you will hit
No spare and no compliant substitute. The stop is cut to what can be delivered, the office tells the customer before the crew leaves, and untouched tasks are scheduled rather than dropped.
It runs again once it cools. An intermittent thermal fault is still a fault, so the tag stays on and reads "ran again after about 20 minutes cool-down", the most useful line a shop tech gets.
