Wet Floor and Slip Hazard Control
Purpose
The person a wet floor hurts is almost never the cleaner. It is the occupant, the customer walking in off a rainy sidewalk, the older parent crossing a hallway at night. What makes it different from every other hazard in the trade: the person at risk did not see the work happen, has no protective equipment, and is moving at normal walking speed on a surface that looks the same as it did an hour ago.
So the controls are a barrier, a route, and a release gate tied to something you can see rather than a timer.
Scope
Covers any process that leaves a walking surface wet or slippery during ordinary cleaning: damp mopping, auto-scrubbing, spray buffing, spill response, entry mats saturated by weather, and the transition edges between a treated and an untreated surface.
Excludes: floor finish chemistry, stripping and recoating, owned by the strip and wax SOP; why a floor is dull, hazy or streaked after service, owned by the floor finish trees; a spill of blood or body fluid, owned by the sharps and body fluid SOP. Note the split in who is protected: 29 CFR 1910.22(a)(2), which requires walking-working surfaces to be kept clean and, so far as feasible, dry, is a duty the shop owes its own employees. The occupant is outside OSHA's reach, and what the crew owes them is the ordinary duty of care, which is why the barrier goes up before the water goes down.

Procedure
Plan the wet route before anything is wetted, so a dry path always exists. Mop away from the exit, split wide areas in half lengthwise so one side stays walkable, and never wet a corridor end to end when it is the only route to a bathroom or a fire exit. Acceptance: from every occupied room, you can point at a dry path to an exit and a bathroom for the whole time the floor is wet. Wrong looks like a cleaner working into a corner and stepping across their own wet floor to get out. Stop rule: a layout with no dry path is cleaned in halves, the second not started until the first releases. Hazard: crossing your own wet work is the cleaner's own slip, and it happens with a bucket in both hands.
Put the barrier up before the water goes down, and size it to the whole wet area. Signs at every approach to the treated area, not one in the middle, and a cone or a physical block at any doorway a person could enter through without seeing a sign. Acceptance: from each entry point in turn, a warning is visible before a foot could land on wet floor, and each face carries a signal word and a wet-floor symbol to ANSI/ASSP Z535.2, the facility sign standard, in the edition your program or the client's contract adopts, which is how it binds. Wrong looks like one folding sign in the middle of a lobby with three doors. Stop rule: not enough signage on the truck to cover every approach means the area is worked in smaller sections that the available signage does cover. Hazard: a sign placed in the walking line is its own trip hazard, so it goes to the side of the path with its faces square to the approach.
Control the water, because a wet floor and a flooded floor are different hazards. Wring to damp, not dripping; on a machine, set solution flow so the squeegee recovers everything in one pass. Acceptance: no standing film anywhere, and no visible water trail behind the mop or scrubber after the pass. Wrong looks like a mop laying water that pools at a low spot or runs under a door into a room nobody signed. Stop rule: standing water anywhere stops the pass; recover it before continuing, because water that has crossed a threshold is now in an unsigned area. Hazard: water reaching a floor outlet, a power strip or an extension cord joint is an electrical hazard, so cords run outside the wet area, nothing is plugged or unplugged with wet hands, and a cord lying in water is not picked up until the circuit is off.
Watch the residue, not just the water, because a dry floor can still be slippery. Over-application, an under-rinsed cleaner or the wrong dilution leaves a film that grabs underfoot when humidity or a damp shoe reaches it. Acceptance: no tackiness under a gloved fingertip and no footprint on the finished floor when someone walks it. Wrong looks like a floor that looks perfect dry and prints every step on a humid morning. Stop rule: any tack or printing means a clean-water rinse pass before release, not a buff to hide it. Hazard: the slip resistance the floor was specified with is a property of the tile, not of your water. ANSI A326.3, binding only where a building code or your contract adopts it, sets a wet dynamic coefficient of friction of at least 0.42 for level interior tile expected to be walked on when wet, and a residue film can put an already-compliant floor under that number without changing how it looks.
Tell the people who will walk it, by room and with a time. Name the rooms, name the dry path, and give a rough release time rather than "in a bit". Acceptance: the occupant or site contact has repeated back which areas are out and how they will get where they need to go. Wrong looks like a crew that signs the floor and says nothing, in a house where someone will come downstairs in the dark. Stop rule: where the occupant cannot be told directly, as at a public entrance or a shared corridor, the barrier does the whole job and is upgraded to a physical block rather than a sign. Hazard: none to the crew; this is the step that protects the person the barrier is for.
Release on an observable, never on the clock. A damp-mopped hard floor with air movement commonly releases in about ten to twenty minutes, so use that to plan, then check before you release: no sheen when you look across the surface at a low angle toward a light, and dry to the back of a hand. Acceptance: both checks pass at the slowest point of the area, which is usually the corner furthest from air movement, not the middle. Wrong looks like a lead releasing a corridor at fifteen minutes because fifteen minutes is what it usually takes. Stop rule: any sheen at the low angle keeps the barrier up, adds air movement, and moves the check out in short intervals until it passes; the release is never split, so a partially dry area stays closed. Hazard: this step decides whether anybody gets hurt, and it is the one under the most pressure from a waiting occupant.
Handle a spill you did not make as its own event, at once. Stop what you are doing, stand a sign over it, and only then go for a mop. Acceptance: the spill is covered by a warning within a few seconds of being seen, and it is cleaned to the same acceptance as step 4. Wrong looks like a cleaner walking past a spill to fetch a mop and returning to find someone standing in it. Stop rule: a spill of something you cannot identify is not mopped; it is signed, isolated and reported to the office for the product identity. Hazard: an unknown liquid may be a chemical or a body fluid, and mopping it spreads whichever it is across a much larger area, so identification comes before the tool.
Take the barrier down as a deliberate step, and put back everything you moved. Signs and cones come out only after step 6 passes, and they come out completely, counted onto the truck against the number that went in. Chairs, mats, waste bins and door stops go back where they were. Acceptance: sign count out equals sign count in, every moved item is on its position, and no mat has a curled corner or a fold. Wrong looks like two cones left in a corridor overnight, which train everyone in the building to walk past a cone. Stop rule: a sign that must stay up because an area is not released is reported to the site contact by name, with who removes it and when, rather than left to become permanent. Hazard: a mat put back with a rolled edge is a trip hazard the crew created while removing a slip hazard, which is the trade this step exists to prevent.
The record this produces
Four fields on the ticket, and they are what a shop needs when someone reports a fall.
- Areas treated, with time wet and time released, per section rather than for the visit as a whole.
- Barrier placement: how many signs or cones, at which approaches, and the count recovered at step 8.
- The release check result at the slowest point, in step 6's words: sheen or no sheen at low angle, dry or not to the back of the hand.
- Who was told, by name, and what dry path they were given.
The office keeps these with the visit, and the owner reads them against any slip report the same day: the first question is not whether the floor was wet, it is whether the barrier was up and the release check was recorded.
Worked pass
Small dental office, evening clean, two staff still on site finishing paperwork in the staff office at the far end, which opens onto a rear vestibule with the staff restroom and the rear exit off it. Hard floor: a reception area of about 360 square feet and a corridor of about 200, so 560 square feet total, worked as two sections in that order.
Step 1 put reception first, mopped from the front door back to the corridor mouth, then the corridor, mopped from the mouth down to the rear vestibule, so the cleaner backed out of each section instead of crossing it and the staff kept a dry path to a restroom and an exit the whole time either section was wet. Step 2 placed signs at the three approaches, corridor mouth, reception door and the door from the staff office, checked from each spot in turn. Step 5 told both staff which sections were out, gave the dry path, and had it repeated back.
Step 3 ran reception from 18:05 to 18:12 and the corridor from 18:12 to 18:20 with no standing water and no trail. Step 4 found no tack and no printing on either section.
Step 6 failed on the first check. At 18:35, fifteen minutes after the last pass, reception read dry at the low angle and to the back of the hand, but the corridor showed a sheen along the stretch furthest from the reception door, where the mop was carrying the most solution partway through that second section. Under step 6's stop rule the barrier stayed up on the whole corridor, an air mover went in at the far end, and the check moved out. The corridor passed both checks at 18:52, seventeen minutes past the planned release, and the release was never split even though reception had passed at 18:35.
Step 8 took the three signs down at 18:53 and counted three back onto the truck, put the two waiting-room chairs and the entry mat back, and confirmed the mat lay flat. The ticket recorded reception wet 18:12 released 18:35, corridor wet 18:20 released 18:52, three signs at three approaches with three recovered, the release check as no sheen at low angle and dry to the back of the hand at the corridor's far wall, and both staff told by name.
The corrective action was a mop-water rule rather than a person: a mop head carries the most solution partway through a section, so it gets re-wrung at the halfway point rather than at the end.
