Burned Receptacle Response and Circuit Condemnation
Purpose
Give the shop one standing answer to a call that reads "the outlet burned up." The device is the symptom. The decision this procedure produces is whether the branch circuit behind it is still fit to energize, and that decision has to be made from what the conductors look like, not from how bad the plastic looks.
Two failures happen without a procedure. A tech swaps the device, restores power, and leaves a conductor whose insulation was cooked four inches back inside a wall. Or a tech condemns a circuit on a scorched face that was really a failed plug on the customer's side, and the shop sells a re-run nobody needed. The first starts a fire weeks later; the second costs the account. Both are avoided by inspecting back from the damage before deciding anything.
Scope
Covers the response to a reported burned, melted, sparking or overheating receptacle or its box, on 120 V and 240 V branch circuits in occupied buildings, through to either a device replacement with the circuit released, or a documented condemnation with the circuit locked out.
Does not cover active fire, which is an evacuation and a call to emergency services. Does not cover the panel-side investigation of a repeated trip, which is the panel infrared survey's ground, or the remediation itself once a circuit is condemned; an aluminum remediation or a re-run is quoted work with its own scope.

Procedure
On the phone, before dispatch: get the circuit off and the occupants clear. Tell the caller to switch off the breaker feeding that outlet, or the main if they cannot identify it, and to unplug nothing that is hot to the touch. If anything is smoking or glowing, or there is a burning smell with no visible source, everyone leaves the building and calls 911 from outside, and nobody opens a wall or a panel looking for it. Acceptance: the caller confirms the breaker is off, or confirms the building is evacuated and the fire service is coming. Wrong looks like a caller describing smoke while standing in the room. Stop rule: an active-smoke call is not a service call until the fire service releases the building.
On arrival, isolate and prove dead before the cover plate comes off. Identify the breaker, switch it off, and lock and tag it under 29 CFR 1910.333(b)(2). Then prove the receptacle dead live-dead-live per NFPA 70E-2021, 120.5, in the edition your employer's electrical safety program has adopted. Acceptance: zero volts hot to neutral, hot to ground and neutral to ground, with a meter that proved itself both sides of the test. Wrong looks like a non-contact tester waved at a plate and called dead. Stop rule: if the device stays live with the breaker off, it is fed from somewhere else; find the second source and do not open the box.
Photograph before you touch anything, and get the whole scene. Device face, plate area and surrounding wall, then the device pulled forward with conductors still landed, then close shots of each termination. Acceptance: images that let someone who was not there see which terminal carried the damage and how far the discoloration extends. Wrong looks like one blurry photo of a melted face. Stop rule: no photographs, no condemnation, because the condemnation is an assertion the shop must support months later when it is the reason someone paid for a re-run.
Read the failure pattern at the device before you read the conductors. Damage centered on one terminal screw or one backstab hole with the strap and face largely intact points at a loose termination. Damage centered on the face and the plug slots with clean terminations points at the plug, the cord cap or the appliance. Melting across the strap between hot and neutral points at a short or a foreign object. Acceptance: one of those three named on the report, with the photo that supports it. Wrong looks like "burnt outlet" as the whole diagnosis. Hazard clause: handle a burned device with gloves and do not blow debris out of the box, because burned thermoset and arc byproducts are an inhalation route, not just soot.
Inspect back from the damage until you reach undamaged insulation, and record the distance. Strip nothing yet. Follow each conductor from its terminal into the box, flex it gently, and look for discoloration, stiffening, cracking or a chalky surface, and for a conductor annealed to a dull, soft feel. Acceptance: a point where the insulation is unquestionably normal, inside the box, with enough conductor left to make up a new termination, which means the free-conductor length the NEC requires at every outlet, generally at least 6 in free with at least 3 in outside the box opening. Wrong looks like heat damage continuing past the cable clamp into the wall. Stop rule: damage past the clamp, or too little sound conductor to make up, sends this to condemnation at step 7.
Check the rest of the circuit before you release anything. Open and inspect every other device on that circuit, because the condition that cooked one termination is usually present at the others. Acceptance: every device inspected and recorded as sound or damaged, with the inspected count matching the count you can trace. Wrong looks like a second receptacle two rooms away showing brown discoloration at a backstab nobody opened. Stop rule: a second damaged device moves the finding from an isolated termination to a circuit-wide condition, and a circuit-wide condition is condemned, not repaired one device at a time.
Make the release-or-condemn call against a written rule, and say it out loud to the customer. Release the circuit only when all four hold: damage confined to the device, sound conductor beyond the damage with enough length to make up, no other damaged device on the circuit, and conductor material and termination method that are compatible and listed. Otherwise condemn. Acceptance: the four conditions each marked met or not met on the report. Wrong looks like a release because the customer needs the bedroom outlet tonight. Stop rule: a condemned circuit gets its breaker switched off, locked, and labelled at the panel, every affected outlet labelled at the device, and the occupant told in plain words which rooms are dead and why.
On a release, make up with listed methods and verify before you leave. New device rated for the conductor material, terminations under screw pressure rather than backstabbed, torqued to the device manufacturer's marked value, box fill re-checked if you added connectors. Acceptance: correct polarity, ground continuous, the device holds a plug firmly, and the circuit carries a test load with no voltage drop at the device beyond what the run explains. Wrong looks like a new device on the same loose backstab habit. Hazard clause: restore power with the deadfront on and stand to the hinge side, not square in front, because a fault you did not find announces itself at the panel.
When the site does not match the assumption
The conductors are pre-1972 solid aluminum branch-circuit wiring. Treat any burned termination on that circuit as a circuit-wide condition regardless of what the other devices look like: the Consumer Product Safety Commission has found homes with that vintage of aluminum branch circuit far more likely to have connections reaching fire-hazard conditions than comparable copper-wired homes. Recognized repairs are a copper re-run, or a repair at every connection using a method CPSC recognizes, such as a special crimped connector installed with the manufacturer's certified tooling or a listed set-screw pigtail connector installed per its instructions. A standard twist-on connector on aluminum-to-copper is not a recognized repair, and CO/ALR marking on a device covers that device's own terminations, not splices.
The customer refuses the condemnation and wants the breaker back on. You do not turn it on. Leave it off and locked if the lock is yours to leave, off and labelled if it is not, get the refusal in writing on the field report in the customer's own words naming the circuit, leave a copy, and notify the office the same day.
The property is a rental and the occupant is not the customer. The occupant gets told what is dead and why, in person, before you leave. The condemnation and the quote go to the owner. Do not let a tenant authorize a release.
The record this produces
One incident record per call: date, technician, address and room; the step 1 intake answers including whether smoke was reported; the breaker number and the lock or tag applied; the step 3 photographs; the failure pattern named at step 4; the step 5 inspect-back distance in inches per conductor; the step 6 device list with a count; the four release conditions each marked met or not met; and the outcome as released, condemned, or condemned-and-refused.
The office reads the outcome to decide whether it is quoting a device or a re-run. An insurer reads the photographs and the inspect-back distance if a claim follows. The next technician reads the device list before opening anything on that circuit. A red tag with no closure date is what the office chases weekly, which is the only reason condemnations get corrected instead of quietly re-energized.
Worked pass: 1968 ranch, bedroom receptacle, evening call
Intake: caller reports a brown scorch mark and a smell, no smoke, no heat now. Dispatcher confirms the bedroom breaker is off. Not an evacuation call, so it dispatches.
Step 2: breaker 14 off, locked, tagged. Meter proves on a known live hall receptacle, reads 0.0 V at the target on all three pairs, proves again.
Step 3: eleven photographs. Step 4: damage centered on the brass-side backstab hole, face largely intact, strap unaffected, so the pattern reads as a loose termination rather than a plug fault.
Step 5 fails. The conductors are solid aluminum. The hot conductor's insulation is dark and stiff from the backstab hole for 4 in, and the discoloration continues past the cable clamp into the wall. There is no point inside the box where the insulation is unquestionably sound, so the acceptance condition cannot be met and the free-conductor requirement could not be satisfied even if it could.
Step 6 confirms the pattern. Nine devices trace to circuit 14. Seven are opened and inspected; two behind a built-in are recorded as not inspected rather than as sound. Of the seven, two more show brown discoloration at backstab terminations with no melting yet.
Step 7: all four release conditions fail. Damage is not confined, no sound conductor remains, other devices are damaged, and the material is not compatible because aluminum conductors are landed on backstabs not listed for them. Circuit 14 is condemned. Breaker off, locked, labelled at the panel, all nine outlets labelled, and the homeowner told directly that the two bedrooms and the hall are dead until it is re-run.
The quote goes out next morning for a copper re-run, with a separate line for assessing the remaining aluminum circuits, because seven inspected devices on one circuit say nothing about the other eleven circuits in that panel. The homeowner books the re-run and defers the assessment. The deferral goes on the record in writing with a ninety-day reminder.
