Electrical Incident Response and Reporting
Purpose
Somebody has just been shocked or burned, and the next four minutes decide two outcomes that have nothing to do with each other. The first is whether that person survives and recovers. The second is whether anyone ever finds out why it happened.
Both get lost the same way. A shop's instinct after a shock is to help the person and then tidy up: close the panel, reset the breaker, get the site back in order, sort the paperwork tomorrow. Every one of those acts destroys the evidence of the fault that just injured somebody, and the fault is still there.
The other thing that gets lost is the victim's own judgment. A person who took a shock at 120 V, feels shaken but fine, and wants to finish the day is describing a symptom, not making a decision. Cardiac rhythm disturbances after electrical contact can present hours later.
Scope
Covers the immediate response, scene preservation, notification and reporting for any electrical contact, arc flash, arc blast or electrical burn involving a person, on residential or light commercial work. Covers employees of the shop; where a customer or a bystander is injured, the medical response is the same and the reporting path is not, since OSHA's duties run to your employees. That case goes to your insurer and your own attorney the same day.
Does not cover the investigation and corrective action that follow once the scene is released, or the pre-work assessments whose failure caused the event, which the arc flash, permit and lockout SOPs own.

Procedure
Do not touch the person while any part of them may still be in contact with the source. Open the disconnect first, or separate them with a nonconductive means. Acceptance: the source is confirmed open at the disconnecting means, or the person is clear of the conductor, before any hand goes on them. Wrong looks like a coworker grabbing an arm, which puts a second victim on the floor and doubles the casualties with nobody left to call for help. Stop rule: if the disconnect cannot be found in seconds, use a dry nonconductive object, a fiberglass hot stick or a dry wooden handle, to move the conductor or the person, and never a metal tool or a wet anything. Hazard: this is the step that kills the rescuer, and the reason is that a person in contact with an energized conductor is themselves at that potential.
Call emergency services and send every electrical contact victim for medical evaluation, whatever they say. Acceptance: EMS is called, and the person does not drive themselves, does not return to work that day and does not decline evaluation on their own say-so. Wrong looks like "it was only 120, I am fine," which is the single most common sentence preceding a bad outcome. Stop rule: the injured person does not get the deciding vote on their own evaluation; the shop's rule is evaluation every time, and it is stated in the handbook so nobody has to argue it on a job site. Hazard: current across the chest can leave a delayed arrhythmia with no visible burn, and entry and exit wounds hide deep tissue damage that looks trivial at the skin.
Freeze the equipment exactly as it is. Nothing is reset, closed, cleaned or removed. Acceptance: the breaker stays in the position it went to, the panel stays open or closed as found, burned parts stay where they are, and a lock and do-not-operate tag go on the disconnecting means under the supervising master's name. Wrong looks like a well-meaning tech resetting the breaker so the customer has lights. Stop rule: anyone who already changed something says so immediately and it is written down as a change, not hidden; a reset breaker nobody admits to is worse than one that is documented. Hazard: an arc flash leaves vaporized metal and burned insulation, and burning PVC releases hydrogen chloride, so nobody sweeps, blows out or wipes the enclosure, and older switchgear may carry asbestos-containing arc chutes that must not be disturbed at all. Ventilate and keep people out rather than cleaning up.
Notify the owner immediately and write down the time the contact occurred, to the minute. Acceptance: the owner is told by phone within minutes, and the ticket carries the clock time of contact, the time EMS arrived, and the time the shop learned of any hospital admission. Wrong looks like a text sent at the end of the day. Stop rule: no reporting clock can be computed from "sometime after lunch," so if the exact time is genuinely unknown, record the narrowest window anyone can attest to and label it as a window. Hazard: none at this step, it is a phone call made away from the equipment.
Make the outside report inside the deadline, and know which deadline applies. Acceptance: under 29 CFR 1904.39 a work-related fatality is reported to OSHA within 8 hours of learning of it, and a work-related in-patient hospitalization, amputation or loss of an eye is reported within 24 hours of learning of it. Note the gate that catches small shops: for hospitalization, amputation and eye loss the event has to occur within 24 hours of the work-related incident to be reportable, and for a fatality the window is 30 days. Wrong looks like assuming a small shop is exempt: the partial exemption at 29 CFR 1904.1 for employers with 10 or fewer employees throughout the prior calendar year exempts routine recordkeeping, not this report. Stop rule: when in doubt whether an admission is in-patient or observation, call the hospital and ask before the clock runs; a late report is not curable. Hazard: none at this step, it is a phone call or an online form.
Collect photographs and witness statements the same day, before anyone has compared accounts. Acceptance: photographs of the equipment as found, the position of every device, the tools in use and the PPE worn; and a separate written statement from each witness, taken individually, in their own words, signed and dated. Wrong looks like one group narrative written by the supervisor. Stop rule: nobody is questioned in a way that implies fault, and nobody is disciplined for reporting; 29 CFR 1904.35(b)(1)(iv) prohibits retaliating against an employee for reporting an injury. Any post-incident testing question is one for your own attorney and your written policy, not a decision made on the day. Hazard: photographing inside a damaged enclosure means standing in front of equipment whose condition is unknown, so shoot from outside the arc flash boundary with the disconnect open and locked from step 3.
Enter the case on the injury log inside 7 calendar days, and keep the equipment out of service until the cause is found and corrected. Acceptance: where the shop is not partially exempt under 29 CFR 1904.1, a recordable case is entered on the OSHA 300 log within 7 calendar days of receiving information that it occurred, recordable if it involved death, days away, restricted work or transfer, medical treatment beyond first aid, or loss of consciousness. Separately, the lock stays on until the fault is identified. Wrong looks like the equipment re-energized because the customer needs it. Stop rule: nobody releases the equipment except the named person in the roles table, and never the crew involved. Hazard: restoration puts energy back into equipment that has already injured someone, so it is re-inspected first, everyone stands clear of the enclosure face, and every protective device on the affected circuits is proven to trip and reset before the space is handed back.
The record this produces
One incident file, opened the same day: time of contact, voltage and equipment involved, what the person was doing, PPE worn, tools in use, who was present, the photographs, each individual witness statement, EMS and hospital times, the outside report with its confirmation number, and who holds the lockout tag.
The owner reads it to build the corrective action. The insurer reads it, and an incomplete file gets rebuilt from memory months later, which is the version that gets contradicted. The supervising master reads the PPE and tools lines against what the pre-entry record said should have been used, because that gap is the whole finding in most electrical incidents.
One incident, run
A 14-employee shop. An apprentice replacing receptacles in a small office contacts an energized ungrounded conductor at 2:15 p.m. Tuesday, is thrown clear, stays conscious, and has a small mark on the right thumb.
Step 1: the lead tech does not touch him. He confirms the apprentice is clear of the box, then opens the branch breaker at the panel. Clear before contact.
Step 2: EMS called at 2:17 p.m. The apprentice says he is fine and wants to keep working. Under the acceptance he does not get that vote. Transported at 2:41 p.m.
Step 3 fails. While waiting for EMS, the lead tech closed the branch breaker to restore corridor lighting. That is a real scene change, because whether the breaker tripped on the fault or held is now unknowable. The stop rule fires in the honest direction: he says so immediately, it is written down as a change with his name and the time, and nothing further is touched. The panel is locked and tagged from that point. What was lost is the breaker's as-found position, the single most useful data point for whether the overcurrent device performed.
Step 4: owner called at 2:22 p.m. Contact time 2:15 p.m. is on the ticket to the minute.
Step 5: the apprentice is admitted for overnight cardiac monitoring at 4:40 p.m. Tuesday, and the shop learns of the admission at 5:05 p.m. Tuesday. Two gates apply. The admission occurred 2 hours 25 minutes after the incident, inside the 24-hour occurrence window, so it is reportable. The 24-hour reporting clock runs from 5:05 p.m. Tuesday, so it expires at 5:05 p.m. Wednesday. The report goes in at 8:20 a.m. Wednesday, which is 15 hours 15 minutes into the 24, with 8 hours 45 minutes to spare. The 8-hour deadline does not apply, since that one is for a fatality.
Step 6: photographs taken Tuesday evening from outside the boundary with the panel locked out. Three witness statements taken individually Wednesday morning, signed and dated.
Step 7: the shop has 14 employees, so the 1904.1 partial exemption does not reach it and the 300 log applies. Overnight admission is medical treatment beyond first aid and the apprentice is off Wednesday, which is a day away, so the case is recordable on both counts. Information received Tuesday 5:05 p.m., so the 7 calendar days close the following Tuesday. Entered Thursday, well inside.
Checking the run against the acceptance clauses: step 5's acceptance names an occurrence window and a reporting deadline, and the run prints a value for both, in hours and minutes, from the same two clock times. The failing step prints the change that was made, who made it and what was lost by it, rather than reporting a clean scene.
