Qualified Person Training Record and Refresh

Purpose

"Qualified person" is the single most misread term in this trade, and the misreading is always the same: people hear it as a rank. It is not. Under 29 CFR 1910.399 it attaches to a person for specific equipment and a specific task, based on demonstrated skills and knowledge of the construction and operation of that equipment and the hazards involved. A licensed master who has never opened a 277/480 V panelboard is not qualified for that panelboard. A third-year apprentice who has been trained on it, demonstrated the work practices and been signed off is qualified for exactly that.

That means a shop cannot answer "is he qualified" with a license number, and a permit, a pre-entry record and an incident file all demand exactly that answer, in writing, by name and by equipment. Every other safety procedure in this shelf has a step that gates on it. This is where the answer lives.

The second thing this exists for is the clock. Qualification decays, and it decays silently: nobody's skills visibly expire, so a matrix built once during a good month drifts out of date over two years while everyone continues to believe it.

Scope

Covers establishing, recording, refreshing and auditing qualified-person status for the shop's own employees on residential and light commercial electrical work, plus the awareness training owed to employees who are not qualified persons but are exposed to electrical hazards. Covers the matrix, the evidence behind each cell, the refresh clock and the field audit.

Does not cover state licensing, apprenticeship registration or continuing-education hours for a license, which are a separate credential on a separate cycle and prove something different. Does not cover the task-level assessments those SOPs own: the arc flash pre-entry record, the energized work permit, or the lockout sequence, each of which reads this matrix rather than re-deriving it.

Procedure

  1. Write down the equipment classes and tasks this shop actually performs, as a fixed list. Acceptance: a list of equipment classes with the voltage and configuration named, for example 120/240 V load centers, 208Y/120 V panelboards, 277/480 V panelboards, meter mains, transfer switches and motor control equipment; crossed with the task types this shop runs on each, such as troubleshooting, replacement, and establishing an electrically safe work condition. Wrong looks like a single column headed "electrical work." Stop rule: equipment nobody on the list is qualified for is equipment the shop does not quote, and that is a sales decision made at a desk rather than a dispatch decision made at 7 a.m. Hazard: none at this step; getting it wrong shows up two steps later as a person assigned to gear nobody assessed them on.

  2. Build the matrix one cell at a time, and record what the evidence for each cell actually is. Acceptance: every cell carries a person, an equipment class, a task, a date, and an evidence type that is one of classroom instruction, observed skills demonstration, or a stated number of supervised repetitions. Wrong looks like a whole row filled from a certificate of attendance, since attending a class is evidence of instruction and not of skill. Stop rule: no cell is opened on self-assessment or on years of service; both are common and neither is what 1910.399 asks for. Hazard: none at this step, it is a desk build, and its value is that nobody has to make this judgment while standing in front of an open panel.

  3. Close each cell with an observed demonstration, signed by the person who watched it. Acceptance: the supervising master watched the person identify and avoid the specific hazards, use the work practices, and where the task includes it, establish and verify an electrically safe work condition with the live-dead-live sequence proved on a known source before and after. The signature carries a date and names the equipment. Wrong looks like a signature added from memory a week later. Stop rule: if the demonstration exposes a gap, the cell stays open and the person works supervised on that class until a second demonstration closes it. Hazard: the demonstration is real work on real equipment, so it runs under the same PPE, boundary and permit rules as any other task, and the observer stands outside the arc flash boundary rather than crowding in to watch.

  4. Set the refresh clock and write down the events that reset it early. Acceptance: every cell carries a next-due date no more than 3 years from the last training, which is the maximum retraining interval NFPA 70E sets, in the edition your shop's electrical safety program names. Retraining also happens, without waiting for the clock, when an audit shows the person is not following the safety-related work practices, when new equipment or procedures introduce hazards they were not trained on, or when a task requires practices they do not normally use. Wrong looks like a calendar with only the three-year dates on it. Stop rule: an expired cell is closed, not stretched; the person is not assigned that equipment until a refresh and a fresh demonstration reopen it. Hazard: none at this step, it is calendar work.

  5. Confirm first aid and CPR coverage on every crew, not just somewhere in the company. Acceptance: at least one person on each crew holds current first aid and CPR training, and the shop can say who it is for today's assignments. Where no infirmary or clinic is in near proximity for treating injured employees, 29 CFR 1910.151(b) requires a person adequately trained to render first aid to be available, which on a residential service call means the crew. Wrong looks like a certificate held by an office manager who is not on site. Stop rule: a two-person energized-work crew with no current first aid and CPR between them does not do the work; a shock victim's outcome depends on the person already standing there. Hazard: none at this step, though note the responder's own first act is still to separate the victim from the source before touching them, which the incident response SOP owns.

  6. Audit field work against the matrix annually, and reopen a lapsed cell properly before anyone works on that equipment again. Acceptance: each qualified person is observed doing real work at least once a year, against the practices their cells claim, with the result written down; NFPA 70E in the edition your program names calls for field work to be audited annually and the electrical safety program itself at intervals not exceeding three years, so check both section numbers in your own edition. A reopened cell requires the same signed demonstration as a new one. Wrong looks like an audit that becomes a performance review, which trains everyone to perform for the observer. Stop rule: an audit finding that a person is not following the work practices triggers retraining immediately, ahead of the clock. Hazard: reinstating a lapsed person puts someone back on energized-capable equipment after a gap, so the first job back is supervised and the supervisor verifies the protective functions on any circuit disturbed still trip and reset before the space is handed to the customer.

The record this produces

One matrix, person by equipment class by task, held with the safety program rather than in a personnel file, since the dispatcher has to read it daily. Each cell: date, evidence type, observer name, next-due date, and status of open, closed or lapsed.

Two supporting records. A training file per person holding the certificates, course outlines and demonstration sheets, because a certificate alone does not say what was covered. And an audit log, one line per observation, naming what was watched and what was found.

The dispatcher reads the matrix before every equipment-specific assignment. The supervising master reads the next-due column monthly, because a refresher booked two weeks out is a scheduling item while a refresher booked the morning it expires is a lost day. Anyone reconstructing an incident reads the cell for that person on that equipment, and it needs to have been written before the incident, not after.

One matrix review, run

A four-person shop in May of year 4: an owner-master, two journeymen and one apprentice. Five equipment classes on the step 1 list. Reviewing the panelboard column.

Step 4 fails on one cell. Journeyman A's 277/480 V panelboard cell was last trained in March of year 1. From March year 1 to May year 4 is 3 years and 2 months, which is 2 months past the 3-year maximum. The cell is lapsed, not "nearly due." Under the stop rule it is closed out, and the reason it matters immediately is that the only other person carrying that class is the owner-master.

Journeyman B's cell on that class was never opened, so nothing lapsed; it is open and stays open, which is a different state and the matrix has to show it as one. The apprentice has three states across three columns, which is the normal picture and the reason a per-cell matrix beats a per-person label.

The consequence lands on the board that afternoon. The 277/480 V lighting panel call has exactly one qualified person available, the owner-master, who is on a service upgrade until Thursday. Under step 4 the call moves to Thursday rather than going to Journeyman A on the strength of having done the same work last year. Step 1's stop rule is what keeps this from being a surprise: this shop chose to quote that equipment class, so it needs at least two people carrying it.

The reopening runs the following week. Journeyman A completes the refresher, then step 3's demonstration on a live 277/480 V panelboard under the owner-master's observation: hazard identification stated aloud, boundary measured, PPE selected to the label value, and the electrically safe work condition established and proved live-dead-live on a known source before and after. Signed, dated, equipment named. New next-due date set 3 years out. Step 6 then puts his first job back on that class under supervision.

Checking the run against step 4's acceptance: the failing cell prints its last training date, today's date and the interval between them in years and months, and names the 3-year maximum it exceeded rather than reporting that it was "out of date."
