Receptacle Replacement and Required Upgrade Assessment

Purpose

A receptacle swap is the smallest job this shop sells and the one most often done wrong, because the tech treats it as like-for-like and NEC 406.4(D) does not. Under that section a replacement inherits the protection and device requirements that apply at that outlet today, in the adopted edition, regardless of what came out of the wall.

This guarantees the tech decides four things before the new device leaves the truck: whether a grounding means is really present, which protections ride along, which device and marking set the case calls for, and whether the box can hold what is going in. It prevents the twenty-minute call that leaves an ungrounded outlet wearing a grounding-type face, which reads correct on a plug-in tester and lies to everyone who uses it afterwards.

Scope

Replacement of an existing receptacle in a dwelling or small commercial occupancy, as a service call or inside a larger scope.

Does not cover a burned or overheated device, which is a condemnation question first (burned-receptacle-response-and-circuit-condemnation); the pre-quote survey across a whole remodel (expanded-gfci-requirement-check-before-a-quote, afci-requirement-check-on-a-remodel); outdoor cover listing (outdoor-outlet-and-in-use-cover-verification); or aluminium branch-circuit terminations (aluminum-branch-circuit-remediation).

Procedure

  1. Triage the reason before treating it as a swap. Ask what the device is doing: loose plug fit, cosmetic, dead, warm, discoloured, or a smell. Acceptance: a stated reason on the ticket that is one of those, plus a yes or no on heat and odour. Wrong looks like "replace outlet" with nothing behind it. Stop rule: any heat, discoloration, melting or burning smell leaves this SOP and runs burned-receptacle-response-and-circuit-condemnation, because a swap over a damaged termination puts a new face on an active fault. Hazard: none at this step, it is a question asked before anything is touched.

  2. Isolate, lock and prove dead before the plate comes off. Open the breaker, lock and tag it under 29 CFR 1910.333(b)(2), written for qualified persons under 1910.332 and 1910.399, then prove the outlet dead live-dead-live per NFPA 70E-2021, 120.5, in the edition your employer's electrical safety program has adopted. Acceptance: 0 V hot to neutral, hot to ground and neutral to ground, on a meter proved on a known live source before and after. Wrong looks like a non-contact tester waved at the plate. Stop rule: a device still live with the breaker open is fed from a second source, and the box stays closed until that source is found. Hazard: the device is live until your own meter says otherwise, and the box may carry a second circuit your breaker did not kill, so treat every conductor as energized until proved, work one hand at a time, and do not brace against a grounded surface.

  3. Determine whether a grounding means is present by looking, not by testing. Pull the device forward with conductors landed and identify what the ground terminal connects to: an equipment grounding conductor in the cable, a metal raceway or box that qualifies, or nothing. Acceptance: the grounding means named on the ticket in words, or "none present" stated outright. Wrong looks like a plug-in tester reading correct over a jumper from the neutral terminal to the ground screw, which is a bootleg ground and reads exactly like a proper one. Stop rule: a bootleg ground is cut out, never reused, and the outlet is treated as ungrounded from there. Hazard: the box may hold a second circuit your breaker did not kill, so re-prove each conductor pair before separating anything.

  4. Determine which protections ride along under the adopted edition. Four questions, each from its own section: does 210.8 require ground-fault protection at this location; does 210.12 require arc-fault protection on this branch circuit; does 406.12 require a tamper-resistant device here; does 406.9 require a weather-resistant one. Acceptance: four yes or no answers on the ticket with the section beside each. Wrong looks like deciding from the device that came out, which reflects the edition in force when the house was built. Stop rule: a question you cannot answer from the adopted text goes to the office before the new device is landed. Hazard: none at this step, it is read from a book.

  5. Select the device and the marking set the case calls for. Where a grounding means exists, a grounding-type device is used. Where none exists, the adopted 406.4(D) text permits a limited set: another non-grounding-type device, a ground-fault type marked "No Equipment Ground", or a grounding-type device supplied through ground-fault protection and marked both "GFCI Protected" and "No Equipment Ground". Acceptance: the chosen option on the ticket, with the legends transcribed from the section text rather than typed from memory. Stop rule: a customer asking for a plain grounding-type device in an ungrounded box is refused in plain words, because it is not one of the permitted options. Hazard: none at this step, it is a selection.

  6. Count the box fill before committing to the device. Add conductor volumes from Table 314.16(B) in the adopted edition: each conductor at its size, all equipment grounding conductors together as one, and each device yoke as two conductors of the largest size connected to it. Compare against the volume marked on the box or from Table 314.16(A), plus any listed extension ring at its marked volume. Acceptance: required and available volume both written in cubic inches, available at or above required. Stop rule: required over available means a listed extension ring or a box change, not harder pushing - a crushed conductor behind a device is a fault waiting for a warm day. In pre-1978 target housing a box change disturbs painted surfaces and liberates lead-bearing dust, an inhalation and ingestion route gloves do not address, so the work is performed under 40 CFR Part 745 Subpart E by a certified renovator with containment and HEPA cleanup where your firm is compensated, and any respirator is worn under a written program per 29 CFR 1910.134. Where nobody on site holds that certification, the box change is quoted, not performed. Hazard: the count is arithmetic on a de-energized box; the hazard here belongs to the remedy, named in the stop rule.

  7. Make up with listed terminations, torque, and apply the markings before the plate goes on. Terminations under screw pressure rather than backstabbed, torqued to the value marked on the device or in its instructions with a calibrated tool, legends applied where they stay visible with the plate installed. Acceptance: polarity confirmed by conductor identification, every termination torqued and marked, legend readable after the plate is on. Wrong looks like the marking stuck to the back of the device where no future reader will see it. Stop rule: no marked torque value and no instructions on site, stop and get them. Hazard: none if step 2 held; re-prove dead if the circuit was restored for any reason in between.

  8. Restore power and prove the protection actually functions. Plate and covers on first, then close the breaker standing clear of the panel face. Then test: operate the ground-fault device's test button and confirm it opens the outlet, reset, confirm voltage, and check that tamper-resistant shutters resist a single-blade probe. Acceptance: the device opens on test and resets, line voltage measured at the face after reset, shutters holding, each result on the ticket. Wrong looks like a device that resets but will not open on its own test button, which means it is protecting nothing. Stop rule: a device that will not trip on test comes back out and is replaced, not left with a note. Hazard: this step puts energy back with the customer in the room - keep them out of arm's reach of the outlet and the panel until every test is finished.

When the site does not match the assumption

The customer declines protection the adopted edition requires. The device does not go back as a plain grounding type: reinstall the original non-grounding type or leave the outlet out of service, take the refusal in writing in the customer's own words, and notify the office that day.

The outlet is on a circuit that also needs arc-fault protection and no listed part exists for that panel. That is a panel-side consequence quoted through afci-requirement-check-on-a-remodel, not something resolved by leaving the outlet unprotected.

The property is a rental and the occupant is not the customer. The occupant is told what changed and what a marked device means; the option choice and any refusal go to the owner.

The record this produces

Per outlet: the triage reason and heat and odour answers from step 1; breaker number, lock applied and proving-meter result from step 2; the grounding means named in words, including any bootleg ground found and removed, from step 3; the four protection answers with sections from step 4; the option chosen and legends applied from step 5; required and available box volume in cubic inches from step 6; torque from step 7; and the test results from step 8.

The grounding finding is the line that matters months later: it tells the next tech, and the customer's home inspector, whether this circuit has an equipment ground anywhere on it.

Worked pass: 1961 bedroom outlet, "plugs fall out", 2020 NEC jurisdiction

Step 1 records a loose plug fit, no heat, no odour, so this stays a replacement. Step 2: breaker 7 open and locked, meter proved on a known live source before and after, 0 V on all three pairs.

Step 3 fails. The plug-in tester used before shutdown read correct, but with the device pulled forward the ground screw carries a short jumper landed on the neutral terminal. That is a bootleg ground, the stop rule fires, the jumper is cut out, and the outlet is treated as ungrounded. The two cables are 14-2 without ground and the box offers no qualifying metal path, so the grounding means is recorded as none present.

Step 4 answers against the 2020 text: 210.8 does not reach a bedroom receptacle, so no location-based ground-fault requirement; 210.12 does reach the bedroom branch circuit; 406.12 requires tamper-resistant here; 406.9 does not apply indoors in a dry location.

Step 5 selects the ground-fault type device marked "No Equipment Ground", transcribed from 406.4(D), and pairs it with an outlet branch-circuit type arc-fault solution at the circuit's first outlet to satisfy the second answer.

Step 6: the box is a 3 by 2 by 2.5 in device box at 12.5 cu in from Table 314.16(A). Fill is four 14 AWG conductors at 2.00 cu in each, so 8.00 cu in, no equipment grounding allowance because none is present, and one device yoke counted as two conductors, so 4.00 cu in. Required is 8.00 plus 4.00, or 12.00 cu in, against 12.5 cu in available, leaving 0.5 cu in. Tight but clear, so no extension ring.

Step 7 lands conductors under the screws at the torque marked on the device and puts the legend on the face where it reads with the plate installed. Step 8: breaker closed from beside the panel, the test button opens the outlet and resets, the face reads 121 V after reset, and the shutters hold against a single-blade probe.
