Underground Utility Strike During Foundation Excavation: Response
Purpose
Step 8 of the utility locate SOP covers the first five minutes after a strike: leave, do not touch a switch, call from clear ground. This procedure covers everything after those five minutes, and it is where a strike either closes as a one-page incident report or turns into a six-week dispute over whether the locate was followed.
The pit stays open the whole time this plays out, usually next to a house whose foundation is mid-repair, and an open pit with a damaged utility in it is its own hazard independent of the original strike. What determines how this resolves is whether the site is secured properly, whether the mandatory damage report actually goes in, whether the internal review honestly asks if the tolerance zone was hand-dug, and whether anyone restarts work on a phone call instead of a written release from the operator.
Scope
Covers the response to a struck gas, electric, water, sewer or communications facility during pier pit, trench or anchor excavation, from the point the immediate safety reaction has run through site security, mandatory reporting, root-cause review, repair coordination, restart authorization and close-out.
Does not cover the immediate on-strike safety reaction itself, which the utility locate SOP's damage-response step owns and which this procedure assumes already happened. Does not cover the locate procedure that precedes excavation, which the same SOP owns in full. Does not determine fault or cost allocation for the damage, which is a claims and legal question for the shop's insurer and attorney.

Procedure
Step 1: Confirm the immediate safety reaction ran, then hold the site as a hazard zone
Verify the crew followed the locate SOP's damage-response step for the facility type struck, then barricade the pit and a stand-off area around it, and keep everyone but the operator's own crew or a qualified contractor out. Acceptance: barricade in place, exclusion zone marked, entry list limited to the operator and named shop personnel. Wrong looks like a crew standing around the open pit discussing what happened while a damaged gas line vents nearby. Hazard: a struck facility can still be releasing gas, carrying current, or leaking under pressure well after the initial contact, so treat the exclusion zone as live until the operator says otherwise, not until it looks quiet.
Step 2: Report the damage within your state's mandated window
Call the one-call center's damage-report line and the facility operator directly. Most one-call statutes require excavator-caused damage to be reported promptly, commonly within one to two hours, but the exact window, and whether it runs from discovery or from when work could safely resume, is set by your state's statute, so confirm your own state's figure rather than carrying another state's number. Acceptance: both reports made, with the report reference numbers and call times recorded. Wrong looks like reporting only to the operator and assuming that satisfies the one-call center's own separate requirement. Stop rule: if either report cannot be confirmed as received, escalate to the supervisor immediately rather than assuming it went through.
Step 3: Do not disturb the damaged facility until the operator releases it
Nobody backfills, straightens, wraps, or otherwise touches the struck facility, and nothing is removed from the pit near it without the operator's instruction. Acceptance: the facility left exactly as found, photographed in place by a named person on the step 1 entry list, using a zoom lens from outside the exclusion zone where the facility type allows it rather than approaching, with the pit otherwise undisturbed near it. Wrong looks like a crew taping a nicked line and backfilling to keep the schedule; damaged coating on a steel gas line or a stressed joint on a water main fails later, away from anyone watching for it. Hazard: a damaged facility under load, whether pressurized gas, water, or a stressed electrical conductor, can fail further while being handled by anyone other than the operator's own qualified crew, so this stop applies even once the immediate danger seems to have passed.
Step 4: Coordinate repair scheduling with the operator directly
The supervisor, not the crew, becomes the point of contact with the operator's dispatch or repair crew, and tracks the scheduled repair against the open pit's condition, since an open, barricaded excavation sitting for days next to an occupied house is its own liability the shop should not let drift. Acceptance: a repair appointment confirmed with the operator and communicated to the customer. Wrong looks like the shop waiting passively for the operator to show up on their own timeline while the customer's yard sits fenced off with no update.
Step 5: Run the internal review honestly, before anyone decides how this happened
Pull the locate package: the ticket, the positive response record, the private locate report and its stated gaps, and the mark photographs, and compare them against where the strike actually occurred. Acceptance: a written finding of one of three causes: the facility was unmarked or mismarked by the operator, it was a private line the private locate reported as a gap or missed entirely, or the tolerance zone was not hand-dug or vacuum-excavated as the locate SOP requires. Wrong looks like a review that stops at "the ticket was valid" without checking whether the crew actually hand-dug inside the tolerance zone at that specific location. Stop rule: where the review cannot determine which of the three applies from the existing record, that gap itself goes into the record rather than defaulting to the answer that puts the least fault on the crew.
Step 6: Notify the customer with the finding, not just the fact of the strike
The supervisor tells the customer what was struck, what the internal review found, and what the repair timeline looks like, distinct from the same-day notification the strike itself likely already triggered. Acceptance: a written update with the review's finding delivered once step 5 closes. Wrong looks like leaving the customer with only the day-of phone call and no follow-up once the cause is actually known; a customer who has to call and ask what happened reads the silence as something being hidden.
Step 7: Verify the physical condition before authorizing restart, not just the operator's paperwork
Once the operator issues a written release stating the facility is repaired and safe, the supervisor does not restart work on that email alone. Send someone to the pit to visually confirm the repair, check that the tolerance zone is re-marked if marks were disturbed by the repair work, and confirm no new hazard, such as standing water or a collapsed pit wall from the delay, has developed while the pit sat open. Acceptance: a physical re-inspection logged, separate from and after the operator's written release. Wrong looks like resuming excavation the moment the release email arrives without anyone looking at the actual pit; a release covers the facility, not the condition of the hole it sits in after several days open. Stop rule: any new condition found in the pit, structural or otherwise, is resolved under the excavation and shoring SOP before excavation resumes. Hazard: this is a person standing at the edge of, or entering, a pit that has sat open for days near a facility the operator has only just certified repaired; treat the wall as unverified and the facility as unconfirmed-dead until this inspection's own findings say otherwise, the same caution applied to the pit on the day of the original strike, not a lesser one because time has passed.
Step 8: Close the incident record and feed the review back to the locate file
Attach the finding from step 5 to the job's utility locate file, and where the review found a locate process gap, whether an operator mismark or a tolerance-zone lapse, route it to whoever owns that process for the shop rather than letting it sit in a closed incident file where the same gap can repeat on the next job. Acceptance: the incident record closed, with the finding cross-referenced into the locate file. Wrong looks like the incident report living only in the job ticket, disconnected from the locate procedure it is meant to inform.
The record this produces
One utility-strike incident file per event: the confirmed immediate-response check, the barricade and exclusion zone log, both damage-report reference numbers and their timestamps, photographs of the undisturbed facility, the operator repair appointment, the internal review's finding on cause, the customer notification, the physical restart verification, and the closed record's cross-reference into the locate file.
The office keeps it because an insurer, the operator, or the state one-call program's enforcement process will ask the same three questions this file already answers: was a valid ticket in force, was the tolerance zone hand-dug, and was the damage reported within the statutory window.
One worked pass
A vacuum excavation crew opening a wall-access trench strikes a shallow communications conduit at about 14 in, inside the marked tolerance zone but at a point where the private locator's report had noted the trace as incomplete. Step 1 confirms the crew already backed off per the locate SOP's damage step; nothing else was touched, and the pit is barricaded within minutes.
Step 2 reports to the one-call center and the operator directly. The center's damage-report line is called first and confirms receipt with a reference number; the operator's own line rings through to voicemail on the first attempt, which under step 2's stop rule escalates to the supervisor rather than being logged as done. The supervisor calls the operator's emergency dispatch number instead, which connects, and both reports are now confirmed inside the window the state's statute sets.
Step 5's review pulls the private locate report: it names this exact conduit as a partial trace, stopped 6 ft short of where the strike occurred, with the customer's signed acknowledgment of the gap already on file from before excavation started. That places the cause as the reported gap in the private locate rather than a tolerance-zone lapse, and the review records it as such along with the fact that the crew was, correctly, hand-digging inside the zone at the time.
The operator's repair crew arrives two days later, splices the conduit, and issues a written release the following morning. Step 7 sends the crew lead back to the pit before excavation resumes: the splice is visually confirmed, but two days of open pit and one overnight rain have left standing water at the bottom, a condition the excavation and shoring SOP requires resolving before anyone re-enters. That stop fires, the water is pumped and the competent person re-releases the pit, and only then does the trench work continue.
Step 6's customer update goes out with the actual finding: a documented gap in a private locate the customer had already signed off on, not a shop error, alongside the two-day delay and its cause. Step 8 routes the finding to the estimator who oversees private locate vendor relationships, since a partial trace on a communications line is worth tracking if it recurs on other jobs from the same locator.
