Rooftop Work Fall Protection and Roof Access

Purpose

A tech going up to a rooftop unit is doing the most routine thing in commercial HVAC and standing in the trade's most common fatal exposure. Routine is exactly why it goes unmanaged: nobody writes a plan for the tenth roof this week, so the decision about edges, skylights and anchorage gets made silently, alone, by whoever is holding the gauges. This procedure moves that decision off the roof and onto a survey done before anyone clips to anything, and it gives the tech an unconditional stop. A shop that does not grant that stop in writing has, in practice, told its people to make the call under schedule pressure.

Scope

Covers access to and work on low-slope commercial roofs for rooftop unit service, diagnostics and repair: ladder and roof hatch access, edge exposure, skylights and roof openings, anchorage, and the weather conditions that stop work.

Does not cover steep-slope residential roof work, which this shop refers out. Does not cover craning or rigging equipment onto a roof, which the changeout SOP owns. Does not cover repair of the roof membrane itself: if you damage it, you report it, you do not patch it.

Procedure

1. Establish which standard governs this task before you leave the truck. The split is by activity, not by building type. Routine service, diagnostics and maintenance on an existing rooftop unit is general industry work under 29 CFR 1910 Subpart D, where the unprotected side and edge trigger is 4 feet (1910.28(b)(1)) and low-slope roofs carry their own banding at 1910.28(b)(13). Installing a new unit, setting a curb or altering structure is construction work under 29 CFR 1926 Subpart M, where the trigger is 6 feet (1926.501). Acceptance: you can say out loud which of the two this ticket is. What wrong looks like: assuming the 6 foot construction number applies while doing a service call, which leaves you unprotected in the 4 to 6 foot band. Stop rule: if the ticket is ambiguous, treat it as the stricter of the two and call the service manager.

2. Set the access and prove it before you climb. For a portable ladder: correct duty rating for you plus tools, level and firm footing, side rails extending at least 3 feet above the upper landing surface, secured at the top, and set at roughly a 4 to 1 ratio. For a roof hatch: key in hand, hatch fully latched open, and a fixed ladder or stair in sound condition. Acceptance: the ladder does not shift under a two-hand shake at the base and the extension above the landing is visible from the ground. Hazard in this step: overhead conductors, a ladder set in front of a door that can open into it, and a ladder on a loose gravel or wet surface. Stop rule: any of those three, and the ladder gets relocated before anyone climbs, not steadied by a helper. Portable ladders are 29 CFR 1910.23 in general industry, 1926.1053 in construction.

3. Survey the roof from the access point before you walk it. Standing at the hatch or ladder top, identify every unprotected edge, each parapet height, every skylight and roof opening, each unit you are working on, and the path between them. Acceptance: for each work position, a measured or paced distance to the nearest unprotected edge, written down. What wrong looks like: walking straight to the unit and deciding about edges later, from the far side where the walk back is the exposure. Hazard in this step: a soft deck, standing water, and a membrane over insulation hiding a weak spot, so walk deliberately and stay off ponded areas. Stop rule: if you cannot see the whole roof from the access point, walk the perimeter first with nothing in your hands.

4. Apply the low-slope banding to each work position and each travel path. Under 29 CFR 1910.28(b)(13), work less than 6 feet from an unprotected edge needs a guardrail system, safety net, travel restraint or personal fall arrest. Work 6 to 15 feet from the edge needs one of those, or a designated area, and only where the work is both temporary and infrequent. Work 15 feet or more needs no system only where the work is both temporary and infrequent and your employer has and enforces a rule prohibiting anyone going within 15 feet of the edge without protection. A parapet only counts as a guardrail if it meets guardrail height, which 1910.29(b)(1) puts at 42 inches plus or minus 3 inches above the walking surface. Acceptance: each work position is banded and the chosen control is named. Stop rule: a low parapet is a trip hazard, not a guardrail; band it as an unprotected edge.

5. Prove the anchorage before you clip to it. An anchorage for personal fall arrest must support at least 5,000 pounds per attached worker, or be part of a system designed, installed and used under the supervision of a qualified person with a safety factor of at least two (29 CFR 1910.140). Acceptance: a certified anchor with current documentation from the site contact, or a written engineered system. What wrong looks like: clipping to a vent stack, a duct, a gas line, a curb, a railing of unknown pedigree, or the RTU itself. None of those are anchors and the RTU is the thing you are servicing. Hazard in this step: an anchor that fails is worse than no lanyard, because you tied off and stopped watching the edge. Stop rule: no documented anchorage means no fall arrest option on that roof; the work moves to guardrail or travel restraint, or it stops and the service manager is called.

6. Cover or guard every skylight and roof opening on your path before work starts. A skylight is a hole. Under 29 CFR 1910.28(b)(3), employees must be protected from falling through holes more than 4 feet above a lower level by a cover, a guardrail or a fall arrest system; the construction counterpart is 1926.501(b)(4). Acceptance: every opening in your work area and travel path covered with something rated to hold the load, secured against displacement, and marked. What wrong looks like: a domed plastic skylight treated as a solid surface, or a weather screen assumed to be a fall cover. Neither holds a person. Stop rule: if you cannot protect an opening, route around it with the route physically marked; if the route cannot avoid it, the work stops.

7. Check weather and surface stop conditions on arrival and again after any break. Acceptance: dry membrane, wind low enough that a removed panel does not become a sail, no audible thunder, and a work and rest plan if the surface is hot. Set a wind number your shop holds to and write it in the site file, because that call made alone under schedule pressure goes one way. What wrong looks like: carrying a panel across a roof in a gust, or working a wet single-ply membrane, which is close to frictionless. Hazard in this step: a dark membrane in summer runs far above ambient, so water, shade at the hatch and a check-in interval are part of the plan. Stop rule: thunder means everyone comes down immediately and stays down for your shop's stated interval.

8. Descend and close out deliberately. Account for every tool and every panel screw before you approach the ladder, secure the hatch, return keys, and tell the site contact the roof is clear. Acceptance: nothing left on the roof, hatch latched, temporary covers either left in place with the site contact told or removed with openings restored. What wrong looks like: a panel left leaning against a curb for the next wind event. Hazard in this step: descending with tools in hand rather than on a hoist line or in a closed bag takes a hand off the ladder. Stop rule: a missing tool gets found before you come down.

The record this produces

One roof access block on the ticket, completed at the access point rather than at the truck: which standard governs (general industry or construction) and why; access means and its condition; distance from each work position to the nearest unprotected edge; parapet height; the control chosen per position (guardrail, travel restraint, fall arrest, designated area, or the 15 foot rule); anchorage used and its documentation reference; skylights and openings found and how they were protected; weather at start and at any restart; and any stop, with the time and who was called.

It also feeds the site file, which is the part that compounds. The second visit to that roof should start from a known edge geometry and a known anchorage situation, not a fresh survey by a tech who has never been there. The service manager reads it to decide whether a site needs permanent guardrail before the shop accepts more work there, and the shop reads it after any incident, when the question will be what was known and when.

Worked pass: strip mall roof, two rooftop units, hatch access

Summer service call, single-story strip mall, two packaged units, roof hatch in a back-of-house corridor. Site contact met the tech with the key.

Steps 1 and 2 passed: routine service on existing units, so general industry, 4 foot trigger and low-slope banding under 1910.28(b)(13), written on the ticket; fixed ladder to the hatch sound, hatch latched fully open. The step 3 survey from the hatch found a rectangular roof with a parapet running the full perimeter at about 12 inches high, two domed skylights between the hatch and the east unit, and no ponding. The west unit sits about 14 feet from the north edge. The east unit sits about 3 feet 6 inches from the east edge, taped rather than paced because it was clearly inside the tightest band.

Step 4: a 12 inch parapet is nowhere near the 42 inches plus or minus 3 that 1910.29(b)(1) requires of a guardrail, so both positions band against an unprotected edge. West unit at about 14 feet falls in the 6 to 15 foot band, and the work is temporary and infrequent, so a designated area was set with a warning line and the work rule stated to the helper. East unit at 3 feet 6 inches falls under 6 feet, which permits only guardrail, net, travel restraint or fall arrest.

Step 5 FAILED. No certified anchor existed on the roof and the site contact had no documentation for anything. The only candidates a tech would be tempted by were a plumbing vent stack and the east unit's own curb, and neither is an anchorage under 1910.140. Under this step's stop rule that removed both fall arrest and travel restraint from the east unit's options, leaving guardrail, and no guardrail was on the truck. The tech phoned the service manager from the roof; east unit work was stopped and rescheduled, and portable weighted guardrail was arranged for the following morning.

Steps 6 through 8 continued for the west unit, which the stop did not affect. Both skylights sat on the path to the east unit and were roped off and marked rather than covered, since no rated cover was available and no work required approaching them. Close-out accounted for tools, latched the hatch, and told the site contact the east unit remained out of service.

What the failure teaches: steps 1 through 4 all passed, and a tech running only those would have reached the east unit having correctly banded it under 6 feet, clipped a lanyard to the nearest solid-looking thing, and worked the call believing they were protected. The banding decision is worth nothing without step 5, and step 5 is the one that most often has no good answer on an older commercial roof.
