Containment Breach Discovered Mid-Job: Response Procedure
Purpose
A breach is not a maintenance note, it is the moment the whole reason for containment stops being true. This procedure guarantees that the instant a breach is suspected, dust-generating work stops, every technician is accounted for, the clean side is assessed for exposure, the breach is repaired to the original material standard, and negative pressure is re-proven with a number before anyone resumes. Skip any of those and the job either exposes an occupant to spores nobody documented, or exposes the shop to a clearance failure it can no longer explain.
Scope
Covers detection and response to a breach in an already-built, already-running containment during active remediation: a torn flap, a failed seam, a register that lost its seal, a chase connecting to unsealed space, or a manometer that will not hold negative. It does not cover initial chamber construction, owned by the containment chamber construction SOP, and not routing personnel between two simultaneously active containments, owned by the cross-contamination prevention SOP. A breach that reveals contamination beyond original scope hands off to the scope-change SOP once this procedure closes.

Before you touch the breach
Full PPE, respirator seated, stays on through detection, assessment, repair, and the resumption check that confirms the gradient is back. A breach means the gradient meant to pull contaminated air away from you has failed, so the air at the boundary is the most contaminated air in the building at that moment, not the least. Do not lift a respirator to talk, and do not send an unmasked technician to "just take a quick look."
The procedure
Stop all dust-generating work the instant a breach is suspected. Hands off tools, demo and HEPA-vacuuming halt immediately, no finishing the current section first. Acceptance: the work area is static within about 10 seconds of the call. Wrong looks like a technician who keeps cutting because they are "almost done with this stud bay." Stop rule: any continued work after the call is a hard stop, escalated to the lead on the spot; this is the single most common way a suspected breach becomes a confirmed exposure event, because contamination keeps aerosolizing every second demo continues.
Call the breach out loud and account for every technician by name. The technician who noticed it calls "breach" audibly or over radio; everyone freezes in place; the lead reads the crew list and gets a verbal reply from each name. Acceptance: every technician in the containment answers within about 30 seconds. Wrong looks like assuming everyone heard because the containment is small. Stop rule: any technician who does not answer halts everything else until they are located, because an unaccounted-for technician may be past the breach point in a chase or void that was not part of the controlled work area.
Locate the breach before repairing anything. Walk the containment boundary with a chemical smoke pencil at every flap, seam, and penetration, starting closest to whatever triggered the call, usually a manometer alarm or a technician feeling airflow. Acceptance: visible outward smoke movement at a specific point, or a visible tear, gap, or dislodged seal. Wrong looks like re-taping a high-traffic corner on a guess without confirming it is the actual source. Stop rule: no clear source found in about 10 minutes means treat the entire boundary as suspect and check every seam in sequence rather than resuming on an assumption.
Assess what and who was on the clean side while the breach was open. Determine the direction the breach vented (toward an occupied room, an adjacent containment, or unoccupied exterior) and whether an occupant or another trade was present downstream during the failure window. Acceptance: a stated location and duration, for example "kitchen, approximately 12 minutes," not "not sure." Wrong looks like skipping this because the reseal felt quick. Stop rule: any occupant or trade present downstream during the failure window triggers mandatory notification in step 8, regardless of how short the window was, and this finding is not closed until step 7 addresses it.
Repair the breach to the original material standard, not a field patch. Patch torn poly with matching 6-mil material overlapping the damage by at least 6 inches on every side, taped both faces; replace a failed flap outright rather than re-taping a tear in it. Acceptance: the patch meets the overlap margin and holds when tugged firmly at each edge. Wrong looks like a strip of tape shorter than the tear, or re-taping a seam without addressing why it failed, such as a missing batten weight. Stop rule: any repair under the overlap margin, or a second failure at the same location, condemns that section of boundary; it gets rebuilt, not patched again. Hazard: you are working at the point of highest airborne concentration in the containment, so PPE stays sealed through this step, no exceptions for a "quick" patch.
Restore and re-verify negative pressure before anyone resumes dust-generating work. Let the AFD run undisturbed through its warm-up window, then take a manometer reading at the repaired boundary per the negative-pressure verification procedure. Acceptance: a reading at or beyond -0.02 in WC (-5 Pa), held steady for at least 60 seconds. Wrong looks like judging the plastic "looks like it's pulling in" instead of reading the gauge. Stop rule, and this is the enforceable check that governs resumption: a reading that has not reached -0.02 in WC, or one that will not hold steady, means work does not resume. A verbal "looks fine" from anyone on the crew does not substitute for the number, because a re-established gradient and a repaired hole are not the same thing to confirm. Hazard: the boundary being read here is the same location the PPE mandate above names as the most contaminated air in the building during a breach; full PPE stays sealed through this step exactly as it did through detection, assessment, and repair, and does not come off until the reading itself clears -0.02 in WC and holds, not once the repair looks finished.
Close out the exposure question step 4 opened. For any surface identified as downstream during the failure window, either HEPA-vacuum and damp-wipe it under the standard antimicrobial protocol, or, if it sits inside an occupied room, treat it as a new finding requiring its own assessment rather than a quick wipe. Acceptance: a documented action taken, or a documented decision to monitor with a stated recheck time. Wrong looks like leaving it unaddressed because the exposure "was only a few minutes." Stop rule: any downstream occupied room does not return to normal use until this decision is documented, and if the surface reveals contamination beyond the original scope, hand off to the scope-change procedure before treating it as routine cleanup.
Log the event and get authorization before demo resumes. Record start and stop time of the stoppage, the breach location and cause, the repair made, the reading from step 6, and the exposure finding from step 4. A routine reseal with no downstream exposure needs a phone call to the office; any occupant or trade exposure needs a same-day customer notification from the project manager, not a mention at project close. Acceptance: a complete log entry exists before tools restart. Wrong looks like resuming from memory and writing the log at day's end, which is exactly how duration and cause get reconstructed wrong later. Stop rule: an incomplete log means the job is not authorized to resume, independent of the reading in step 6. A good manometer number with no log entry is still a stop.
The record this produces
A breach incident log, filed with the project's daily record, not a separate document that can go missing. It carries: time called and time resumed, location and physical cause, the headcount result, the exposure finding from step 4, the repair made, the manometer reading from step 6, and who authorized resumption. The office reads this log before releasing any clearance report, because a clearance sample pulled after an undocumented breach is defensible only if the file shows the breach was found, closed, and re-verified first.
One run of this procedure, filled in
Second-floor bathroom remediation, single containment, occupants staying in the home in a bedroom down the hall.
- Step 1: a technician feels airflow reversing at the containment's outer flap mid-demo. Work stops within seconds.
- Step 2: breach called out loud. Headcount confirms both technicians present and clear of the boundary.
- Step 3: smoke pencil finds a supply register in the hallway wall that lost its poly seal, apparently jarred loose when a ladder brushed it.
- Step 4: the register vents into the hallway, not a bedroom. Estimated open duration is about 8 minutes based on when the airflow reversal was noticed against when demo started that block.
- Step 5: the register is resealed with fresh 6-mil poly and duct tape, full overlap on all sides.
- Step 6: first manometer reading after warm-up comes back at -0.008 in WC, under the -0.02 in WC floor. Fail, stop rule taken. No resumption. The crew re-walks the boundary and finds a second, unrelated issue: a flap corner batten had come untied.
- Step 6, repeated: batten reset, second reading holds at -0.03 in WC for the full 60 seconds. Passes.
- Step 7: the hallway is HEPA-vacuumed and damp-wiped along the section nearest the register, documented as a precaution rather than a confirmed contamination event, since the exposure window was short and outside any occupied room.
- Step 8: full log entry completed, project manager calls the homeowner the same afternoon to explain the event and the precautionary cleaning, since a trade or occupant standard applies even though no one was actually in the hallway at the time.
The second leak is the point worth keeping. A single confirmed reseal that still fails its own pressure check is not a fluke reading, it is evidence the first fix did not address everything that was wrong, and the stop rule exists precisely so nobody talks themselves into accepting a number that is close enough.
When the response does not go as written
Found by an AFD alarm rather than a person. Run the same procedure from step 1; the stop and headcount still come first, since an alarm tells you pressure dropped, not that everyone is accounted for.
The technician is solo. Step 2 becomes a self-check: stop, confirm you are clear of the boundary, then proceed to step 3. Do not attempt the repair alone if it needs poly held while taping; wait for a second person or use battens and clamps instead.
The downstream side is another active containment. Steps 4 and 7 still apply, but route the finding through the cross-contamination procedure instead of an occupant notification, since that containment's own log governs from there. If the breach instead reveals a void outside the mapped work area, stop at step 3 and open the scope-change procedure before any repair decision, since sealing it without disclosing what it exposed just re-hides the finding.
