Moisture and Conducive Condition Report
Purpose
Conducive conditions are the reason retreats exist: water reaches wood, wood stays wet, and the colony killed last year has the same invitation this year. The report is the only part of that the shop controls, and it fails the same way every time - a line reading "moisture present in crawl" that cannot be compared to next year, handed to a plumber, or used to justify anything.
This SOP guarantees the opposite: every finding leaves the site with a measured number at a named location, a source, an owner and a date.
Scope
Covers the moisture survey and conducive condition inventory on residential and small commercial structures: exterior grade and water management, crawlspace and basement conditions, wood moisture readings, and the classification and referral of each finding.
Does not cover crawlspace entry, gated by the crawlspace inspection and treatment standard. The wood destroying insect report form and its legal effect belong to that SOP, which this report feeds numbers to; soil termiticide geometry belongs to the liquid termiticide standard and carpenter ant moisture work to the carpenter ant SOP. This report never diagnoses a plumbing, roofing or structural defect; it measures, locates and refers.

Procedure
Step 1 - Check the meter and fix the correction before a single reading is taken. Read the manufacturer's calibration check block or reference resistor with the pin meter, then look up the species and temperature corrections for the framing and the working temperature. Acceptance: check reading inside the manufacturer's stated band, and the net correction in points written on the ticket with the species and temperature it was read at. Wrong looks like raw readings recorded as moisture content, which run off by whole points and make this year's numbers uncomparable to last year's. Stop rule: check reading outside the band and no readings are taken with that meter; use the backup or rebook. Hazard: a pin meter's pins are a puncture route and carry whatever they were last driven into, so they stay sheathed between readings and are wiped before going back in the case.
Step 2 - Walk the exterior for the water source before you go under anything. Follow the water: roof to gutter to downspout to discharge to grade to foundation, then add irrigation heads, hose bibs, condensate lines and any mulch or siding against the wall. Acceptance: each defect photographed with its elevation and a compass direction, downspout discharge distance from the wall measured, and grade called as falling away or not at each elevation. Wrong looks like a report naming poor grading with no elevation, leaving the customer's landscaper nowhere to start. Stop rule: an active leak with running water is reported to the customer before you go further, and the shop operates no valve beyond an accessible fixture stop, and then only when asked. Hazard: gutters are read from the ground or from a ladder set at four to one and tied off per 29 CFR 1910.23, never by leaning off the top rung, and dogs are confirmed secured before the walk starts.
Step 3 - Enter the crawl or basement only through the gate that owns it. Run the atmosphere test, the standing-water and electrical screen and the clearance measurement exactly as the crawlspace inspection and treatment standard writes them, then number the bays on your sketch. Acceptance: that SOP's entry criteria all met and recorded on this ticket too, so the report stands alone as evidence, and every bay numbered before any reading is taken. Wrong looks like a survey done from the hatch with a flashlight and reported as a crawl inspection. Stop rule: any entry gate in that SOP fails, nobody enters, and the report is issued with the bays logged as not reached and why. Hazard: standing water touching any electrical equipment means nobody enters and nobody reaches in, the circuit is opened at the panel first by a qualified person under 29 CFR 1910.333(b)(2), and if nobody on site is qualified the space stays closed until an electrician has been.
Step 4 - Take wood readings at fixed points and print the correction on every one. Read the band joist, the sill at two piers, the subfloor under each wet-area fixture and any stained member, at points you mark and photograph so the next tech finds them. Acceptance: each point recorded as raw reading, correction applied and corrected value, all on the same species and temperature basis, with the shop bands applied to the corrected number only - below 16 percent normal, 16 to under 20 percent elevated and monitored at the next service, 20 percent and above an action finding with a named source. Wrong looks like a single reading taken at the wettest spot and reported as the crawl's moisture content. Stop rule: any point where an awl enters across the grain more than a quarter inch under moderate thumb pressure is a structural referral, photographed, not probed further and not described as repairable. Hazard: meter pins driven into framing can reach concealed cable, so no pinning within a hand's width of a device box or a visible cable run and none at all near knob and tube; use the pinless meter there and note it.
Step 5 - Inventory the conducive conditions against a fixed list, not from memory. Every time: wood to ground contact, form boards left in place, siding or stucco below grade, insufficient ground clearance, a torn or missing ground vapor retarder, blocked or insufficient under-floor ventilation, debris and stored lumber, condensate or dryer discharge into the space, standing water. Acceptance: every item marked present or absent with a bay number, plus a ventilation calculation showing under-floor area, the ratio set by the International Residential Code edition your jurisdiction has adopted - commonly 1 square foot of net free vent area per 150 square feet of under-floor area, reduced to 1 per 1,500 only where a Class I vapor retarder covers the ground continuously - and the net free vent area found, taken from the vent manufacturer's rating rather than the frame size. A retarder torn open in one bay is not continuous, so 1 per 150 governs that crawl until it is repaired. Wrong looks like an inventory of what the tech happened to notice, which is why the same house gets three different reports in three years. Stop rule: anything you lift or open to take a measurement, including the ground vapor retarder and any vent cover, is lapped back in its original direction, weighted or re-secured, and verified before you leave that bay. Hazard: crawling to a reading point puts your face against soil, insulation and droppings, so heavy droppings are wetted before disturbance per the attic rodent cleanup SOP, and unidentified loose granular insulation is not touched at all.
Step 6 - Classify each finding and put a name on it. Sort every finding into monitor, correct, or refer, and assign an owner: the shop, the customer, or a named trade - plumber, roofer, grading contractor, structural engineer. Acceptance: no finding left unclassified, every correct or refer item carrying an owner and a target date, and every monitor item carrying the reading that will be compared at re-read. Wrong looks like a findings list where everything is a recommendation and nothing has an owner, which reads to the customer as a sales sheet. Stop rule: a finding you cannot classify goes to the service manager the same day rather than being written as a general observation. Hazard: none at this step, it is sorted at the tailgate, though stay out of the drive lane on a road frontage.
Step 7 - Issue the report so next year's tech can re-run it. Deliver the findings list with the photographs, reading table, ventilation calculation, owners and dates, and set the re-read date on the account. Acceptance: customer signature or a dated delivery record, declined items logged with dates, and a re-read scheduled at the next regular service or sooner where an action finding is open. Wrong looks like a report delivered verbally at the door, which leaves the shop with nothing when a retreat is disputed. Stop rule: the customer declines an action-threshold finding, log the refusal with its measured value and issue the report anyway. Hazard: none at this step, though a report naming a possible structural defect is a referral to an engineer, not an opinion the tech signs.
The record this produces
Per reading point: bay and member, raw value, the correction with its species and temperature basis, corrected value, band, and a photograph of the marked point. Per site: meter check result, exterior defects with elevation and direction, the conducive-conditions list, the ventilation calculation, findings with owner and target date, refusals with dates, and the re-read date.
The service manager reads open action findings against retreat history: a retreat on a house with an unresolved reading at or above 20 percent is a correction problem, not a product problem.
Worked pass: 1,260 square foot crawl, four bays, six reading points
Step 1 fails first: the pin meter reads 19.2 percent on a check block stated as 18.0 percent plus or minus 0.5, outside the band. The stop rule fires, that meter goes back in the truck flagged, and the backup reads 18.1 percent on the same block, inside the band, so the survey runs on it.
Framing is southern yellow pine, the crawl holds at 58 F, and the meter's species and temperature table gives one net correction of minus 1.5 points for all six points.
Six readings, raw then corrected: band joist north 22.4 to 20.9; subfloor under the hall bath 24.8 to 23.3; sill at pier 3 18.1 to 16.6; sill at pier 7 15.0 to 13.5; joist 12 14.2 to 12.7; band joist south 13.8 to 12.3. Against the bands applied to the corrected values, two are action findings at 20 percent and above, one is elevated in the 16 to under 20 range, and three are normal below 16 percent, accounting for all six.
The awl at the hall bath subfloor enters an eighth of an inch under thumb pressure, short of the quarter inch trigger, so it stays a reading rather than a structural referral.
Ventilation: 1,260 square feet of under-floor area, with an intact Class I ground vapor retarder in three bays and the sheet torn open across bay 2. Four vents at a rated 50 square inches net free area each give 200 square inches installed. Because the retarder is not continuous as found, 1 per 150 governs: 1,260 divided by 150 is 8.4 square feet, or 1,210 square inches required, against the 200 installed. As found the crawl is under-ventilated by roughly a factor of six, and that is the finding the report leads with. Repairing bay 2 changes which ratio governs: with the retarder continuous, 1,260 divided by 1,500 is 0.84 square feet, or 121 square inches required, which the installed 200 clears. So the retarder repair is written as the correction that restores compliance, not a cosmetic item.
Findings and owners: the hall bath supply weep to a plumber with a target date; the north downspout discharging 8 inches from the wall to the customer with an extension length named; the torn retarder in bay 2 to the shop at the next visit; the sill at pier 3 a monitor item at 16.6 percent. The customer declines the downspout, logged with the date, and the re-read is set at the next quarterly service.
