Water Heater Failure Investigation and Warranty Claim
Purpose
Every piece of evidence a warranty claim needs sits on site until the tank goes on the truck, and then it is gone. The claim is decided by what was recorded before that, and the first question is not whether the unit is in warranty but what actually failed, because a shell failure, a fitting failure and a relief valve doing its job are three findings with three different payers. This procedure also forces the shop to find and price the condition that killed the tank, so the replacement does not follow it.
Scope
Covers the investigation visit on a residential or small-commercial storage or tankless water heater that has leaked, flooded or failed, and the evidence package and claim submission that follow.
It does not cover the diagnostic call for no hot water, which routes here when it finds a shell leak; the changeout itself, which the water heater changeout SOP owns; or water damage restoration, which is its own trade and its own handoff.

The handoff that breaks: the tech swaps the tank the same afternoon, the old unit goes to the scrap bin, and the claim is submitted from memory a week later with no serial photograph and no static reading. That claim is not weak, it is unsupportable.
Procedure
Make the site safe before any investigation, and do it in the order the hazards rank. If there is standing water touching an electric appliance or its circuit, nobody touches the appliance or metal contacting it and the branch circuit is opened from a dry location; if anyone smells gas, everyone leaves immediately, nothing is switched, no phone used inside, and the call to the utility is made from outside under the gas odor SOP. Acceptance: energy isolated, water isolated at the cold inlet, and the floor structure under a full tank assessed, because 50 gallons of water alone is about 417 pounds on whatever the tank is standing on. Wrong looks like starting the investigation with the appliance still energized in water: stop. Hazard: these are the hazards, and each carries its action above.
Locate the failure before you name it, by drying everything and watching. Dry the shell, the pan, every fitting, the relief outlet, the drain valve, the condensate connection on a hybrid and any line overhead, then leave it and come back. Acceptance: one origin identified, the wet path traced from it and photographed while wet. Wrong looks like calling a shell failure because there is water under the tank: relief discharge, a weeping nipple, a leaking drain valve, condensate and a sweating cold line all wet the same floor, and only one is a tank claim. Hazard: reaching behind a tank on a wet floor means gloves, a light rather than a hand, and the breaker or gas cock confirmed off from step 1.
Date the unit from its own label and get the purchase and install evidence in the same visit. Acceptance: serial photographed legibly, the manufacturer's date coding decoded for that brand, and either an install record from the shop's file or the customer's proof of purchase captured. Wrong looks like using the customer's memory: the serial is the only date the manufacturer will accept, and shops routinely find a unit a year newer or older than everyone believed. Hazard: none, this is reading and photographing, but do not lean on the flue connector to reach the label.
Measure and photograph every site condition the manufacturer's terms turn on, whether or not you like the answers. Acceptance: cold static in psi at a hose bib with nothing running, the recovery peak against the relief valve's marked set where the unit can still be run, closed or open as a yes or no, relief and discharge disposition, expansion control present or absent, any softener, any anode record, and whether a residential unit is serving a commercial load. Wrong looks like recording only the conditions that help: a missing condition reads to a reviewer as one nobody checked, and the ones that hurt are exactly what the shop needs to price a correction for. Hazard: thread the gauge fully home before opening the bib, because a gauge blown off at high static is a projectile.
Build the evidence package to the manufacturer's stated claim requirements before anything is disconnected. Acceptance: the photo set complete against that manufacturer's published list, which typically includes the rating plate, the leak location, the whole installation, the relief discharge and the surrounding connections, plus the step 4 readings written in. Wrong looks like four dark photographs at an angle: reshoot them, because a reviewer who cannot read the plate has nothing to approve. Hazard: none, but keep the customer clear of the wet area.
Get an authorization or claim number before the unit is disposed of, and hold the unit for the period the manufacturer states. Acceptance: claim opened, a reference number recorded, and the disposal instruction obtained in writing, whether that is return the unit, hold it for a stated period or scrap it. Wrong looks like scrapping the tank the same day: many programs require the failed unit be available, and a scrapped tank ends the claim however good the photographs were. Hazard: none, a phone call and a note.
Tell the customer what the shop can actually promise, and refuse the thing it cannot. Acceptance: the customer told, in one conversation, that the shop will submit the evidence and correct the cause, and that whether the claim is allowed is the manufacturer's decision under their own written terms, which typically cover the tank or its parts and commonly exclude labor and prorate after a period. Wrong looks like "you're covered, don't worry": what a warranty document obliges is a question about that contract and, if contested, one for the customer's own attorney, so hand them the terms rather than your reading of them. Hazard: none, a conversation, and the step most often skipped.
Replace, correct the cause, and return to service with the protective function proved. Acceptance: the new unit installed to the changeout SOP, every condition step 4 found out of order corrected or quoted as its own line, the replacement registered, and a full recovery watched to a normal thermostat cutout with a fixture within about 5 F of target. Wrong looks like a new tank on the same closed system at the same static with no expansion control: the shop has just installed the next claim. Hazard: this step puts fuel or electrical energy back with the customer in the house, so confirm the tank is full to a solid stream for 60 continuous seconds before energizing, run a spillage test at the draft hood about a minute after a gas burner fires with a carbon monoxide monitor on you, and close an electric breaker from the hinge side.
When the claim will not go the customer's way
Failure is a fitting, nipple or drain valve rather than the shell: usually a parts or workmanship question and often a repair, so do not open a tank claim on it. The unit is out of term: say so plainly on the first visit rather than submitting to be seen trying. A condition the terms exclude is present: submit anyway with it disclosed, because concealing it turns a declined claim into a lost customer, and quote the correction either way. Customer wants the tank gone today: photograph everything, get the claim number the same day, and store the unit yourself if the manufacturer requires it held.
The record this produces
- Both site-safety gates and the isolation performed
- Failure origin named, with wet-state photographs and the observation interval
- Serial photographed and the decoded manufacture date, plus purchase or install evidence
- Cold static in psi, recovery peak against the relief valve marked set, closed or open, expansion control present or absent
- Relief and discharge disposition, softener present, any anode record on file
- The photo set checked item by item against the manufacturer's claim requirement list
- Claim number, date opened, and the written disposal or retention instruction
- Corrections made or quoted, the replacement's serial and registration, and the recovery cutout observed
The photo set and the static reading are the whole claim. Everything else can be reconstructed later, and those two cannot.
Worked pass: 50 gallon gas tank leaking, five years into a six year term
Basement utility room, water spreading across the slab, no hot water, no gas odor. Customer believes the tank is about four years old.
- Steps 1 to 3: gas control to OFF, cold inlet closed, no electric appliance in the water, slab floor so no structural concern. Everything dried and left 90 minutes; water reappeared at the lower shell only, with rust staining above it consistent with a long seep rather than a sudden event, while the relief outlet, drain valve and all fittings stayed dry. Origin named as a shell failure and photographed wet. Serial decoded to a manufacture date 5 years and 4 months back against a 6 year tank term, so the unit is in term and 16 months older than the customer thought.
- Step 4: FAILED on three conditions, and all three were recorded anyway. Cold static 96 psi at the hose bib with nothing running, no pressure reducing valve anywhere, which is above the 80 psi threshold in International Plumbing Code 604.8 in the adopted edition. Metered check makes the system closed and there is no expansion tank, so the relief valve at its marked 150 psi set had been the only release for five years. No anode record on file and the customer confirms it was never checked. Stop rule taken on the coverage conversation, not on the work: nothing was promised to the customer about the outcome.
- Steps 5 to 7: photo set built to this manufacturer's published list, twelve images including a legible rating plate, the wet shell, the whole installation, the relief discharge and the meter with its check. Claim opened by phone the same afternoon, reference recorded, written instruction received to hold the failed unit for 30 days. Customer told the shop is submitting the full evidence including the pressure findings, that the decision belongs to the manufacturer under their terms, and that those terms commonly exclude labor and prorate after a period; the terms document itself was sent rather than summarized further.
- Step 8: new tank installed under the changeout SOP. Pressure reducing valve set to 65 psi, expansion tank pre-charged to that same 65 psi under the expansion SOP, new relief valve on a rebuilt discharge under the T&P SOP, each quoted as its own line, replacement registered. Recovery watched: thermostat cut out with the kitchen tap at 121 F against a 120 F setting, and the recovery peak read 71 psi against 65 psi static, a 6 psi rise inside the expansion SOP's 10 psi gate. Anode check scheduled at 2 years.
The three conditions that failed step 4 are why this visit was worth doing properly. Whichever way the claim goes, the shop found and priced what killed a five year old tank, and the customer's next one is not on a closed system at 96 psi with a relief valve doing an expansion tank's work.
