Drain Cover and Entrapment Compliance Check
Purpose
A drain cover is not a grate. It is a certified assembly of a cover, a sump and a flow rating, and it expires. Shops fail this three ways: they replace a broken cover with whatever fits, they never read the life date stamped on it, and they upgrade a pump without re-checking whether the flow now exceeds what the cover was certified to pass. Each leaves a pool that looks finished and is not compliant, against a hazard that kills children and disembowels them.
This procedure guarantees every suction outlet is identified, each cover's marking is read rather than assumed, the flow through it is measured at the worst case a customer can create, and a single blockable main drain has a working second layer. It produces a dated compliance sheet the operator can hand an inspector.
Scope
Covers the compliance check on pools, spas, wading pools, and vanishing-edge or surge systems: outlet inventory, cover marking and life, cover-to-sump match, fastener condition, flow verification, single-outlet layered protection, and the written finding.
It does not cover replacing a cover or retrofitting a layered device, which are quoted repairs done under the manufacturer's instructions, nor general suction-side plumbing faults, which the equipment diagnostic articles own. Federal duty here runs to public pools and spas under the Virginia Graeme Baker Pool and Spa Safety Act; residential pools are reached only where your state or local code has adopted an equivalent requirement, so confirm the local rule before telling a homeowner they are out of compliance.

Procedure
If any cover is missing, cracked, or loose, stop the pump and close the pool before doing anything else. Shut the circulation pump at its disconnect, clear the water, and physically prevent entry with a barrier or a locked gate. Accept the closure when the pump is off and nobody is in the water. Wrong looks like finishing the inspection first because the pool is busy. A missing or broken cover over a live suction outlet is the exact condition that eviscerates a child, and it can do it in the time it takes to walk the deck. Never reach into an open sump to feel for damage with the pump running.
Inventory every suction outlet on every body of water, including the ones nobody counts. Main drains, spa drains, wading pool drains, surge tank and vanishing-edge suction, in-floor cleaner suction, and skimmer equalizer lines. Accept when every outlet is on the sheet with its body of water and its plane, floor or wall. Wrong looks like a sheet listing two main drains at a facility whose spa has its own pair and whose skimmers carry equalizers. An outlet nobody inventoried is an outlet nobody checks, and equalizer lines are missed most often because they sit behind a skimmer throat.
Read the marking on each cover and transcribe it, do not summarize it. Manufacturer, model, the standard and edition it certifies to, the certified flow rating, the installed orientation that rating applies to, and the life or expiry date. Accept when all six fields are on the sheet for every outlet. Wrong looks like a marking abraded to illegible: an unreadable cover cannot be shown to be in date or in rating, so it fails and is replaced. Never enter the water to read a marking while the pump runs; shut it down first, because the same suction that holds you also holds your hand.
Confirm the cover matches the sump it is installed on, not just the bolt pattern. Certification is granted to a cover in a specific sump configuration and orientation, so a floor-rated cover on a wall outlet, or a cover fitted to a field-built sump it was never tested in, is uncertified even when it bolts up. Accept when the installed sump matches what the cover's instructions permit. Wrong looks like a cover sitting proud, rocking, or bridging a sump larger than its listed range. Fail it and quote cover and sump together, because ordering the cover alone repeats the fault.
Check the fasteners and the seat. Every fastener present, the manufacturer's type, tight, and not corroded through. Accept when no cover lifts, rocks, or rotates under firm hand pressure with the pump off. Wrong looks like two of four screws, a stainless screw in a brass insert eaten by galvanic corrosion, or a cover held by silicone. Any of those fails the outlet and the pool stays closed until it is corrected; a cover that comes loose under load is the same hazard as no cover at all.
Measure the flow through each suction line at the worst case a customer can create, and compare it with the cover's certified rating. Valve the skimmers closed so the drain line carries everything the pump can pull, run the pump at its highest programmed speed, and read the flow meter. Where two outlets share a line, each cover must carry the FULL line flow, because either one can be blocked. Accept when the measured line flow is at or below every cover's certified rating for its installed orientation. Wrong looks like a pump upgraded since the covers went in, which is the most common way a compliant pool quietly stops being one. Fail it, and correct by limiting the pump's programmed maximum and re-measuring, or by replacing the assembly with a higher-rated one, never by leaving a note on the file.
Determine whether any body of water has a single blockable main drain, and identify its second layer. A single outlet a body can block must be paired with one of the recognized alternatives: a safety vacuum release system, a suction-limiting vent system, a gravity drainage system, an automatic pump shut-off, or drain disablement. Accept when the device is present, identified by make and model, and listed for this application. Wrong looks like a single drain with nothing but a certified cover. That fails, and the interim measure is disabling the drain and running on skimmers, not leaving it live while a part is on order.
Test the layered device by its manufacturer's procedure, not by feel. For a safety vacuum release system, follow the maker's method for inducing the release and confirm it shuts down or vents within the stated response. Accept when the device performs on two consecutive tests and both are recorded. Wrong looks like a device that trips once and not again, or one whose test button is used as proof of function when the manual specifies a flow-blockage test. Fail it. Never block a live outlet with your hand to induce a trip; use the tool the manufacturer specifies.
Close out in person with the operator and leave the sheet. Walk the failed lines with them at the pool, not at the office. Accept when the operator has signed and the pool's status, open or closed, is written on it with a time. Wrong looks like a report emailed that evening while the pool stayed open on a failed outlet. If the operator declines to close a pool with a failed outlet, record their name, the time and their stated decision on the sheet, hand them a copy, and notify the service manager the same day.
The five entrapment hazards, and what each control actually addresses

One control covers more than one row, which is why partial fixes do not work. Replacing a cracked cover with an uncertified one closes the evisceration row and leaves the hair row open.
The record this produces
The entrapment compliance sheet, one per body of water per inspection, attached to the property and kept across inspections so the expiry dates are visible in sequence.
Fields per outlet: body of water, location and plane, cover manufacturer and model, standard and edition, certified flow rating, installed orientation, expiry date, sump match, fastener condition, measured line flow with the conditions it was measured under, and pass or fail with the reason. Per body of water: single-outlet status, layered device make, model and both test results, pool status at departure with the time, operator's signature.
The operator hands this to the health inspector. The service manager reads the expiry dates to schedule replacements before they lapse rather than after. The next technician reads the measured flow to know whether a pump change is still inside the covers' ratings, which is the check nobody remembers to run at the time of the swap.
Worked pass: a facility pool whose pump was upgraded last season
Commercial pool with two floor main drains on a shared line, two skimmers with equalizer lines, and an attached spa with its own pair of floor drains.
Step 1: all six covers present, none cracked or loose on a pass from the deck, so no immediate closure.
Step 2: inventory captures six outlets, and the two skimmer equalizer lines make it eight. The operator's own records listed four.
Step 3: markings transcribed for all eight. The pool's two main drain covers carry a certified rating of 96 gpm in the floor orientation and a life date still two seasons out. One spa cover's marking is abraded and unreadable, so it fails under step 3's rule and the spa is closed.
Steps 4 and 5: sumps match on the pool pair, fasteners complete and tight. One pool cover has a stainless screw in a brass insert with visible corrosion, noted for replacement at next service but still tight and not a fail today.
Step 6 FAILS on the pool main drain line. Skimmers valved closed, pump at its highest programmed speed, flow meter reads 118 gpm. Each cover on that shared line must carry the full 118 gpm because either can be blocked, and each is certified for 96 gpm. Under the stop rule the correction is made on site rather than noted: the variable speed pump's programmed maximum is reduced and the flow re-measured at 92 gpm, at or below the 96 gpm rating, and the new maximum is locked in the controller. The pool re-opens on the reduced setting, with the sheet recording both the 118 and the 92 and the reason the ceiling exists. Replacing both covers with an assembly rated at or above 118 gpm is quoted so the facility can get its flow back.
Step 7: the pool has two drains, so no layered device is required. The spa has two as well, but it is already closed on the unreadable cover.
Steps 8 and 9: no layered device to test. The operator signs, the spa is recorded closed with the time, the pool recorded open on a locked pump ceiling, and the corroded fastener plus the two cover quotes go to the service manager the same day.
