Utility Line Near-Miss Response Standard

Purpose

This standard covers the job that was never scoped as line-adjacent: the tree everyone measured as clear, until a branch swings farther than expected, a section drops off-line, or a rope brushes a service drop nobody was watching because the primary line twenty feet away had all the attention. It guarantees that the sixty seconds after an arc, a contact, or a too-close pass end with every person and every piece of equipment at a safe distance, not with a crew finishing the cut they already started. Without it, the instinct under adrenaline is to grab the branch off the wire, retrieve the saw, or decide the line "looks dead" and keep working, and that instinct is what turns a near miss into a fatality.

Scope

Any moment during active work, cutting, rigging, climbing, or ground handling, when a saw, rope, ladder, branch, or falling section makes or nearly makes contact with an overhead line that was not part of the job's planned scope. Covers primary distribution, secondary service drops, and transmission lines. A near miss on a shared pole where a communication or cable line hangs below power conductors is covered here too; do not wave it off as "just the cable line" without confirming which conductor was actually involved.

This standard does not cover the pre-job coordination and go or no-go decision for a tree already known to be near lines before the crew arrives, which the utility line clearance reference owns in full; that ground is not repeated here. It does not cover a felled section striking a structure, which the sibling structure-strike standard owns.

Stop before anything else moves

Treat every conductor as energized regardless of what type it appears to be or what the customer says about it. If a saw, rope, ladder, or section is touching or resting on a line, leave it there; nobody goes back for equipment. If a person is in contact with the tree, rope, or equipment that is touching the line, do not touch them to pull them free, a rescuer who makes contact becomes a second casualty with nobody left to call for help. Call 911 and the utility's emergency line, describe exactly what is contacting what, and wait for a fire crew or the utility's own line crew to separate it.

The procedure

  1. Call "line" the instant contact, an arc, or a too-close pass is seen or heard, and everyone stops moving toward the tree. The person closest kills their saw if it can be reached without approaching the line, otherwise leaves it running and backs away. Acceptance: zero crew members advancing toward the tree once the call is made. Wrong looks like someone stepping closer to see what happened. Stop rule: anyone who advances toward the tree after "line" is called is pulled off the job for the remainder of that day, not as discipline but because an adrenaline response that advances once will do it again. Hazard: ground current spreads outward from a contact point, and a person walking normally toward it can bridge a voltage gradient between their two feet and complete a circuit through the body.

  2. Back everyone to at least the OSHA 1910.269 minimum approach distance for the highest-voltage conductor on that structure, or farther if the voltage is not known. That floor is 10 feet at or below 50 kV, rising with voltage class, per the full table this shelf's utility line clearance reference carries. Acceptance: every crew member and every piece of equipment they are carrying at or beyond that distance. Wrong looks like backing away six feet and calling it done. Stop rule: an unknown voltage is treated as the highest class present on that pole, and the crew backs away farther, not closer, until told otherwise. Hazard: a limb still settling in the canopy can fall into or across the line as the tree resettles, so the crew stays clear of the tree's own fall radius as well as the approach distance.

  3. Account for every crew member by name, and ask each one specifically whether they touched the contacted equipment. Acceptance: a full headcount plus a clear yes or no on contact for every person, not a general "everyone's up and talking." Wrong looks like clearing someone because they're standing and responsive. Stop rule: anyone who touched the equipment, or who cannot say clearly that they did not, goes to 911 as a possible electrical injury regardless of how they present, because a current path through the body can leave someone conscious and mobile for a short window before cardiac arrhythmia sets in. Hazard: internal injury from a current path is not visible from outside; appearance does not clear anyone.

  4. Call the utility's emergency line every time this SOP is running, and add 911 if a person is down or equipment remains in contact. A clean near-miss, an arc or a too-close pass with nothing actually touched and nobody hurt, still gets the utility call; that line stayed live and the crew's own read of "nothing happened" is not the same as the utility confirming the conductor's actual state. Add 911 alongside it if a person is down or equipment remains in contact; the line down but the area clear of people still gets the utility call alone at minimum. Acceptance: the utility called on every activation of this SOP within the first few minutes, 911 added when either condition applies, with the utility's ticket or reference number written down. Wrong looks like treating a near-miss with no contact as a story for the tailgate rather than a call, or calling the office first and the utility as an afterthought. Stop rule: if the utility's emergency line is not answered promptly, 911 dispatch can relay to the utility, so do not sit on hold while equipment stays energized in the tree. Hazard: none new at this step; it is a phone call made from beyond the approach distance already established in step 2.

  5. Hold the exclusion zone at the approach distance until the utility confirms de-energization or arrives on scene, not until the line looks dead. Acceptance: the zone held with a name assigned to enforce it, until the utility gives explicit clearance. Wrong looks like someone drifting back toward the tree once the initial adrenaline passes. Stop rule: a line that looks dead is still treated as live, because distribution circuits carry automatic reclosers that re-energize a line without warning, sometimes more than once, and only the utility's confirmation changes that status. Hazard: an automatic recloser cycling the circuit back on while someone is standing where the crew's own judgment said it was safe.

  6. Leave any contacted equipment exactly where it is until the utility gives clearance; do not retrieve a saw, rope, or ladder from the tree or the line. Acceptance: the equipment left in place with its location noted for the utility crew. Wrong looks like sending someone back for the saw once the initial shock of the moment passes. Stop rule: retrieval happens only after the utility has confirmed the line de-energized and cleared the scene explicitly, never on the crew's own read that it looks safe now. Hazard: the same step-potential and recloser hazard from steps 1 and 5, and this is the moment a crew most often talks itself into going back in.

  7. Document the event the same day: what was being cut, what contacted the line, wind conditions, the utility ticket number, and photos shot from beyond the approach distance. Acceptance: a written account and photo set completed before the crew leaves site, with each person's role in the moment recorded. Wrong looks like a vague summary written days later from memory. Stop rule: if the utility or an inspector requests a statement and none exists yet, the crew lead writes it immediately rather than reconstructing the sequence a week out. Hazard: none; this step happens from a safe distance after the scene is cleared.

  8. Get written resume authorization from the office, and re-measure the actual clearance from the remaining canopy to the line before the next cut. Acceptance: a specific recorded answer, resume, reschedule pending utility clearance, or refer the remaining canopy to a line-clearance-qualified crew, with its reason. Wrong looks like picking the saw back up once the utility truck leaves because "they said it's fine now." Stop rule: work does not resume in the same footprint until someone confirms the remaining structure clears the approach distance with margin, because a re-measured, still-too-close reach is exactly what caused the near miss. Hazard: the tree's remaining structure may have shifted from the event itself, changing where the next cut's reach actually lands.

The record this produces

What happened and when, the utility's ticket or reference number, the headcount and per-person contact check, the photo set, and the resume decision with its basis. This is the account the utility, the insurer, and the customer all get, instead of three different secondhand versions.

Worked pass: a service drop nobody was watching

A three-person crew is pruning a maple twenty feet from a primary distribution line they measured and flagged that morning. A ground worker walks a cut branch toward the chip pile; its far end, still attached higher in the canopy, brushes a secondary service drop feeding the house, a line nobody had been tracking because all the attention was on the primary. A visible arc, no one touching anything at the moment of contact.

Step 1 fails on its second half. The worker who dropped the branch calls "line" and steps back correctly, but the chipper feeder, on instinct, moves about four feet toward the branch to pull it clear of the wire before the crew lead shouts him back. Under the stop rule, that crew member is pulled off the job for the rest of the day, not as punishment but because an adrenaline response that advanced once toward a live contact is treated as a real repeat risk, not a one-time lapse.

Steps 2 through 8 pass. The remaining two back to the full ten feet, confirm no one touched the branch or the wire, call the utility and note the ticket number, hold the zone until the utility crew arrives twenty-two minutes later and confirms the drop de-energized, leave the saw and the branch in place until cleared, and document the sequence before leaving site. The office authorizes resume the following morning, after a re-measurement shows the remaining canopy clears both the primary and the now-repaired service drop with margin.
