Ladder + Scaffold Safety Reference

Why this matters

Falls are the #1 cause of death in construction (OSHA 2024). Ladder + scaffold misuse + missing fall protection take more lives than any other workplace hazard. The 30 seconds spent setting up correctly saves a career-ending injury OR a death. This is the field card.

OSHA ladder requirements (29 CFR 1926.1053)

Inspection before use: cracks, missing rungs, damaged rails, worn feet, oily steps.

Set up on stable surface: not on ice, mud, soft soil. Use leg leveler if uneven.

Angle (extension ladder): 4:1 ratio. 4 ft up = 1 ft out from base. Ladders too steep tip backward; too shallow tip forward.

Extension above landing: extension ladder rails extend 3 ft above the upper landing surface (roof edge).

3-point contact at all times: 2 hands + 1 foot OR 1 hand + 2 feet. Don't carry load that requires both hands.

No standing on top 2 rungs of stepladder OR top 4 rungs of extension ladder.

Tie off the top OR have helper hold base if ladder over 20 ft OR on slick surface.

Ladder types + ratings

Type IAA: 375 lb load. Extra-heavy commercial / industrial.

Type IA: 300 lb load. Heavy commercial.

Type I: 250 lb load. Commercial / professional.

Type II: 225 lb load. Light commercial / professional.

Type III: 200 lb load. Household / DIY (avoid for service trades).

Get Type IA OR better: pro service techs + tools + materials easily exceed 200 lb.

Ladder materials

Aluminum: lightweight, doesn't rot. CONDUCTS ELECTRICITY (never use near live wires OR overhead power lines).

Fiberglass: non-conductive (required for electrical work). Heavier, more expensive.

Wood: traditional, non-conductive. Heavier; warps + degrades.

Specific ladders for trades

Stepladder (A-frame): 4 ft - 12 ft; freestanding.

Extension ladder: 16 ft - 40 ft (closed length 8 - 20 ft); reaches high.

Articulating ladder (Little Giant): configurable A-frame + extension + scaffold base. Versatile.

Multi-position ladder (Werner): similar to articulating.

Platform stepladder: extra-stable for prolonged work; bigger platform.

Attic ladder: built-in pull-down (usually 8 ft, sometimes 10 ft).

Scaffold types + use

Single-tier rolling scaffold: 6 - 10 ft height. Workspace 5 ft × 7 ft typical. Common HVAC + ceiling work.

Multi-tier rolling scaffold: stacked frames; 12 ft - 30 ft height.

Frame scaffold: traditional fixed scaffold; tied to building.

Pump jack scaffold: extends along wall; siding, painting, roofing.

Boom lift / scissor lift: motorized; rentable.

Scaffold safety (29 CFR 1926.451)

Capacity: 4x rated maximum intended load.

Decking: solid platform; no gaps over 1".

Guardrails: on a scaffold erected after Jan 1 2000, top rail 38" - 45" above the platform; mid-rail roughly midway, about 21"; toeboard at least 3-1/2" high. Note the general construction guardrail rule in 1926.502 uses a slightly different top-rail band, 42" plus or minus 3", so quote the range that goes with the standard you are actually working to.

Cross-bracing: required per frame.

Outriggers / stabilizers: required for height > 4x base width.

Fall protection: required > 10 ft (per OSHA construction standard).

Inspection: by competent person before each shift.

Fall protection (above 6 ft - construction; 4 ft - general industry)

OSHA 1926.501: fall arrest required > 6 ft (construction).

Three components:

  1. Anchor point: rated 5,000 lb; structural connection
  2. Harness: full-body, NOT single-point belt (causes internal injury)
  3. Connector: shock-absorbing lanyard OR SRL (self-retracting lifeline)

Lanyard length: such that worker falls < 6 ft AND doesn't strike lower level.

SRL (self-retracting lifeline): stops fall within 3 - 4 ft. Used in tight spaces.

Roof anchor types

Permanent anchor: bolted into rafters; stays in place. Solar + recurring service.

Temporary anchor: nailed into rafters; removed at end of job.

Reusable anchor: clamped to structure; transferable.

Anchor point must be ABOVE worker when possible (limits free-fall distance).

Common ladder accidents + causes

Ladder slip-out at base: insufficient angle OR slippery surface.

Ladder tip-back: too steep OR over-reaching at top.

Ladder slip sideways: foot off rail OR weight shift.

Falls from top: standing on top 2 rungs, over-reaching.

Electrical contact: aluminum ladder + overhead power.

Set-up on uneven ground: ladder tips during use.

Carrying ladder solo: drop on head; pinch fingers in extension.

Common scaffold accidents

Wheels not locked. Decking gaps. Missing guardrails. Overloading capacity. Working in wind > 15 mph elevated. Not tying multi-tier to building.

Setup checklist (ladder)

  1. Inspect: no damage
  2. Surface: stable, level (use leveler if needed)
  3. Angle: 4:1 ratio
  4. Top extends 3 ft above landing
  5. Tie off OR have helper if > 20 ft OR slick surface
  6. Tools in tool belt OR pull up with rope (not carried up)
  7. 3-point contact at all times

Setup checklist (scaffold)

  1. Inspect: cross-bracing tight, decking solid, wheels lock
  2. Level: shimmer if needed
  3. Outriggers OR ties: per height
  4. Guardrails + toe boards
  5. Access ladder included (not climbing frame)
  6. Wheels locked when working at height
  7. Fall arrest above 10 ft

Working on roof

Slope class:

  • Low slope (≤ 4/12): standard fall protection
  • Moderate (5/12 - 7/12): slip-resistant boots + fall protection
  • Steep (8/12+): roof brackets + fall protection + harness mandatory

Roof brackets: adjustable steel brackets nailed through the roofing into rafters, carrying a plank across several of them. Several manufacturers make them; buy on the plank width and pitch range you actually work.

Roof jacks: temporary peg + plank system; older standard.

Toe boards: nailed below where you stand; catches slip.

Cold + wet weather

Ice / wet conditions: don't work on roof OR steep ladder; reschedule.

Boots: slip-resistant, lugged.

Gloves: maintained dexterity; not bulky on rungs.

Common ladder + scaffold mistakes

Wrong ladder type rating (consumer light-duty for pro service). Skipping inspection. Working alone above 20 ft (no one to call help). Set up wrong angle. Reaching too far sideways (over-reach tip). Tools at top with no belt (drop on head). Skipping fall protection thinking "it's a quick job". Aluminum ladder near power lines. Standing on top step. Scaffold without guardrails. Scaffold over-loaded. Working in wind.

OSHA enforcement

Fall hazard violations: top-5 most-cited every year (OSHA TOP 10).

Penalty per violation: the serious and other-than-serious maximum is set by 29 CFR 1903.15 and inflation-adjusts every January under the Federal Civil Penalties Inflation Adjustment Act Improvements Act of 2015. Pull the current figure from OSHA's published penalty table rather than working off last year's number.

Repeat / willful: ten times the serious maximum, adjusted on the same January cycle. One willful citation costs more than a full year of the fall-protection gear that would have prevented it.

Criminal charges possible for fatality from willful violation.

NEVER work above 6 feet without proper fall protection (harness + anchor) - even for a "quick" job. Roofs + ladders + ledges that look stable have killed thousands of workers. The 5 minutes to anchor + harness + connect is the cheapest insurance you'll ever buy. A fall from 12 feet to concrete will likely paralyze OR kill. The job will still be there in 5 minutes; you might not be. Fall protection is not optional. Set up + use it every time, regardless of how confident you feel.
