The Tool Condition Inspection SOP
Purpose
To keep unsafe and unfit tools out of the hands of the crew by defining who inspects what, on what trigger, what happens the moment a defect is found, and who decides whether an item is repaired or retired.
The failure this exists to prevent is specific: a defective tool that everybody knows about and nobody removed. That tool gets picked up by whoever needs it next, usually the person least likely to know its history.
Scope
Applies to every item on the tracked register, with three inspection layers running at different triggers:
- Pre-use, by the person about to use the item, on every shift the item is used.
- Post-event, by the holder, after any drop, impact, overload, heat exposure, water exposure, or electrical fault.
- Periodic bench, by the tool keeper, on a cadence set by duty class.
Applies to shop-owned equipment and to personally owned power tools, ladders, and access equipment brought onto company work, because the employer carries responsibility for the safe condition of tools employees use regardless of who bought them.
Consumables are out of scope. So is calibration, which measures accuracy rather than condition and runs on its own schedule (see related).
Roles and responsibilities

Procedure
1. Run the pre-use inspection before the item is used, every shift
Not weekly, not "when it feels off." The classes below get looked at before first use each shift they are used:
- Ladders and access equipment. Rails, rungs and steps, feet and shoes, spreaders, locks, rivets and connections, labels. If you find a cracked or bent rail, a loose or missing rung, a damaged foot, or a failed lock, tag the ladder "Do Not Use" and withdraw it from service immediately, because OSHA 29 CFR 1926.1053(b)(16) requires a defective ladder to be marked or tagged and taken out of service until it is repaired.
- Powered tools. Cord and plug condition including the strain relief, guard presence and function, the switch returning positively to off, housing integrity, chuck or blade retention, and any brake. Pull the tool if a guard is missing or defeated, if insulation is cut or heat-glazed, or if the switch does not return to off on its own.
- Test instruments. Instrument, leads, cables, probes and connectors, inspected for external defects and damage before use, which OSHA 29 CFR 1910.334(c)(2) requires, plus the functional verification covered in its own procedure.
- Lifting, pulling, and rigging gear. Wear, deformation, cuts, heat damage, legible rating markings. Anything with an illegible or missing rating tag is withdrawn, because a rated item you cannot read the rating on is an unrated item.
The pre-use inspection is a look and a feel, not a strip-down. It should take well under a minute per item. If it is taking longer, the standard is written wrong and it will stop happening.
2. Inspect after any event that could have damaged the item
A drop from height, an impact, an overload or bind, a heat event, water immersion, or an electrical fault such as a short or an arc all trigger an inspection before the item goes back into service. For ladders this is explicit: OSHA 29 CFR 1926.1053(b)(15) requires inspection after any occurrence that could affect safe use, not just on a schedule.
The reason this layer exists separately: a periodic inspection can be up to a quarter away, and an item damaged the morning after one is in circulation the whole time.
Post-event inspection is done by the holder, at the time, before the next use. Not at the end of the day and not by somebody else later.
3. Run the periodic bench inspection on the duty-class cadence
The bench inspection is the one that catches slow degradation the eye stops seeing on a tool it handles daily.

Bench inspection is deeper than pre-use: full function through the range, cords flexed along their length, guards and brakes tested, cases checked against their content lists, fasteners checked, labels and rating markings confirmed legible.

4. Tag and withdraw, in that physical order
The moment a defect in the safety-critical list is found, the item comes out of service. Not at the end of the job, not after the current call.
Take the item out of service before anything else: disconnect the battery pack or unplug and coil the cord, bleed and vent any pressurized line, release any spring, blade, ratchet or spring-loaded mechanism, confirm the item is at rest and not energized, pressurized, or holding stored energy - then tag it, then physically remove it from the truck or the shelf into the withdrawal area. An item retired only in the register stays in circulation for whoever picks it up next.
The tag carries four things: the date, who withdrew it, what the defect is, and "Do Not Use." A tag that says only "broken" gets ignored by someone who thinks they know better.
The withdrawal area is a specific, physically separate location, not a corner of the same shelf. Items in mixed storage get taken back.
5. Decide disposition on a rule, within 5 working days
Repair only when all three are true: the repair restores the item to full manufacturer specification, more than 40% of its planned service life remains, and this is not a second repair for the same fault. If any one fails, retire it.
Three separate conditions, all required. Each is there for a reason: a repair that leaves the item working but off-spec is not a repair; a repair on an item near end of life buys weeks; and a second repair for the same fault means the fault is the design or the duty, not the incident.
Two overrides sit above the rule:
- A ladder or item of access equipment is repaired only with manufacturer-approved parts and instructions. Where the manufacturer provides no repair for that component, the item is destroyed so it cannot be retrieved and reused, and then disposed of. A withdrawn ladder that goes in a dumpster intact comes back out.
- Anything whose defect involves a protective function - guard, brake, interlock, insulation, rating marking - retires unless the manufacturer's own repair restores that function fully. There is no partial credit on a protective function.
Non-safety defects follow the same 5-working-day clock, and the item stays tagged and out of the truck for the whole of it. "It still works, I will use it until we sort it out" is the exact behavior the withdrawal area prevents.
6. Record every inspection outcome against the item ID
Four fields: item ID, date, inspection layer (pre-use, post-event, bench), result. Defects add what was found and the disposition.
The record is not paperwork for its own sake. It is what step 7 reads, and it is the only way to see that the same item has been repaired twice for the same fault, which is one third of the disposition rule.
7. Review monthly, and read the layers against each other
The owner reviews the month's defects and dispositions, then asks one question that matters more than the defect count:
Was any safety-critical defect found at a bench inspection of a type that the pre-use check covers? If so, that is a pre-use inspection failure, and it gets addressed with that truck's crew that week - regardless of how good the overall numbers look.
A bench inspection is supposed to catch slow drift. When it catches an obvious defect, the layer above it is not running.
Worked example: one quarterly bench inspection
Truck 4's kit comes to the bench: 22 standard-kit items plus 8 home-assigned, 30 items in total.
Results: 24 pass clean, which is 24 of 30, or 80%. Four have minor defects: a case missing an adapter, a worn cutting surface, a stiff adjustment mechanism, a rating label worn to unreadable. Two are safety-critical: a ladder with a loose rung and a powered tool with a damaged cord jacket at the strain relief.
The two safety-critical items, immediately. The ladder is tagged "Do Not Use" and pulled. The powered tool is unplugged, the cord coiled, tagged, and pulled. Both go to the withdrawal area before anything is written down.
Disposition, the ladder. The manufacturer's documentation provides no field repair for a rung on this style of ladder. That triggers the override in step 5 outright: it is destroyed so it cannot be pulled back out of a dumpster and reused, and then disposed of. Note the rule never even reaches the three-condition test, because the override sits above it.
Disposition, the powered tool. A cord replacement is available as a manufacturer part and would restore it to full specification, so condition one clears. It has had no prior repair for this fault, so condition three clears. Condition two: it is 3.2 years into a 4-year hard-duty planned life, leaving 0.8 years, which is 20% of the 4-year planned life remaining. That is below the 40% gate, so condition two fails.
All three are required, so the item retires despite a perfectly good repair being available. This is the rule doing its job rather than being overridden: a cord replacement here buys under a year on an item already scheduled for the replacement cycle, and it consumes the keeper's time and the bench slot to do it.
Disposition, the unreadable rating label. It looks minor and it is not. An item whose rating cannot be read is treated as unrated, so it is withdrawn until the manufacturer's replacement marking is fitted. If no replacement marking is available, it retires.
The finding that outranks all six defects. Neither safety-critical defect appeared in any pre-use report from truck 4 over the preceding quarter, and both were the kind a shift check covers: a loose rung and a visibly damaged cord jacket. That triggers step 7. The response is not a memo. It is standing at the truck with that crew, running the pre-use check on the remaining items together, and finding out whether the check is being skipped or being done without knowing what to look for. Those have different fixes, and the number of defects found tells you nothing about which one you have.
What would change the read. If both defects had been slow-degradation types - a brake losing bite gradually, a cutting surface worn past spec - step 7 would not fire, because those are exactly what a bench inspection exists to catch and a shift check reasonably misses. The trigger is the defect type, not the defect count.
