Active Chimney Fire Evidence Found During Inspection: Response Procedure
Purpose
A technician on a routine sweep or Level 1 inspection sometimes discovers that a fire is currently smoldering somewhere in the system, or finds clear evidence a fire happened recently that nobody reported. This procedure guarantees the crew stops, gets clear, and treats the finding as a life-safety event before it becomes a documentation exercise. Skip it and the failure mode is not usually panic, it is momentum: a tech mid-pass with a brush finishes the stroke, a roof crew keeps working because nobody told them to stop, or a chimney gets swept and handed back to the customer over damage nobody looked at closely enough to see.
Scope
Covers what the crew does in the minutes after discovering, during a scheduled sweep or inspection, that a fire is or was burning in a system with no reported fire event on file. Does not cover selecting which NFPA 211 inspection level applies to a known puffback, purchase, or post-fire trigger call, which is owned by the Which Inspection Level First decision tree, and does not cover the general homeowner-facing chimney-fire-in-progress response, owned by the Creosote Levels reference. This procedure owns the technician's own first actions and the on-site chain through lockout and escalation.

The procedure
Recognize the cues and stop the tool in the flue immediately. Unusual heat radiating from the chimney breast or flue opening beyond what recent use explains, a crackling or roaring sound from inside the flue, a hot creosote smell with nothing currently lit, or embers and glow visible at the cleanout or top all count. Acceptance: the brush, whip, or camera cable is withdrawn the way it went in, no forward or rotational pressure applied after the cue registers. Wrong looks like "one more pass to see how bad it is." Stop rule: any single cue halts that tool in that flue for the rest of the visit, no exceptions for finishing a stroke. Hazard: forcing a cable or brush past sudden resistance in a hot or active flue can dislodge burning creosote onto the tool or push the tool directly into flame; gloves do not stop a flash from the opening.
Get everyone clear and confirm a headcount before anything else. Call or radio any crew on the roof or ladder down and back at least 25 feet from the structure and clear of the roofline drop zone; move the customer and any bystanders the same distance. Acceptance: a verbal headcount confirms every crew member on the callout is accounted for and outside that radius. Wrong looks like assuming everyone heard the call because you did. Stop rule: if headcount cannot confirm everyone clear within roughly a minute, stop and physically locate the missing person before any other step proceeds. Hazard: falling embers, a spalled crown section, or roof debris coming down during a rushed withdrawal.
If any cue suggests the fire may still be active, call it in and evacuate the structure. Increasing heat, visible glow, or smoke actively emerging means 911, everyone out of the structure, no attempt to fight it. Close the damper on the way out only if the control is reachable without putting a hand or face near the firebox opening; and if that does not hold, skip it and keep moving. Acceptance: the call is placed and everyone is outside at the cleared distance. Wrong looks like stepping back in "just to check." Stop rule: any visible flame or heat that is climbing means full evacuation with no re-entry until the fire department or a confirmed cooldown check clears it. Hazard: smoke inhalation and structural fire spread; a hand near the firebox opening during an active event sits exactly where flue gas and embers vent hardest under the fire's own draft.
Before anyone re-approaches, confirm the heat is actually dropping, not just less alarming than a minute ago. Read the exterior chimney breast masonry against room ambient. A reading more than roughly 10 degrees Fahrenheit above ambient that has not measurably narrowed on a recheck a few minutes later means treat it as still active. Acceptance: two timed readings, room ambient noted both times, with the gap either closed to near zero or clearly and repeatedly narrowing. Wrong looks like one reading and a guess that it is cooling. Stop rule: a gap that holds flat or barely moves on recheck keeps the evacuation in place and the fire department engaged; do not let the crew back in on an unconfirmed trend. Hazard: residual embers deep in creosote can reignite after appearing to settle, especially with fresh air reaching them once a damper or cap is disturbed.
Once cleared, lock the appliance out in writing before doing anything else. Tag it out of service on site, verify no residual glow with a camera pass rather than a hand or flashlight alone, and tell the customer not to operate it until cleared. Acceptance: a signed or witnessed do-not-operate notice, dated and naming the specific appliance, left with the customer the same visit. Wrong looks like a verbal "hold off for now." Stop rule: no signed or witnessed notice on file by end of visit means the office follows up in writing by the next business day so a paper trail exists either way.
Photograph every cue before any further cleaning disturbs it. Glazed or blistered creosote, a warped damper, discolored or cracked tile, and scorched cap paint all get documented in place, with location noted by flue and section, before a brush or vacuum touches that area again. Acceptance: dated photos of each visible cue plus a written location note. Wrong looks like sweeping first to "see what's under it." Stop rule: nothing gets brushed, vacuumed, or otherwise disturbed in the affected section until it has been photographed.
Escalate the inspection level rather than clearing on a visual look, then reseat anything you opened. This finding is a mandatory Level 2 trigger at minimum, scheduled before the appliance is cleared for any use, escalating to Level 3 if the camera shows a defect that cannot be evaluated without removal (see the Which Inspection Level First and CSIA Level 2 vs Level 3 references for the trigger detail this procedure does not re-derive). Reinstall any cleanout door or access panel opened during the initial look and verify it seats and moves through its full range. Acceptance: Level 2 scheduled, and the panel confirmed to open and close fully with no gap. Wrong looks like "it looked fine when I glanced in" as the closing statement, or a panel left loose for the next tech to deal with. Stop rule: no Level 1-only clearance is issued when active-fire evidence was found, and a panel that will not reseat correctly is logged as its own new finding rather than left ajar.
The record this produces
A fire-evidence file: timestamped thermal readings, dated cue photographs with location notes, the signed do-not-operate notice, the escalated inspection scheduling confirmation, and a note on panel or damper condition after reinstallation. This is the file the office pulls if the customer calls back with questions, if an insurer asks what was found and when, or if the Level 2 tech needs to know exactly what triggered the escalation before they arrive.
One run of this procedure, filled in
Annual Level 1 sweep on a wood-burning masonry fireplace, no reported issues on the work order. Room ambient reads 68 degrees Fahrenheit.
- Step 1: mid-pass, the second technician's whip meets unusual resistance and the lead tech notices heat off the chimney breast well above what a cold flue should show. Whip withdrawn immediately, no further pressure applied.
- Step 2: crew called down from the roof access point, headcount confirms both technicians and the homeowner clear of the structure within about a minute.
- Step 3: no visible flame or active smoke, so full evacuation and 911 are not triggered at this point; the crew stages 25 feet back rather than re-entering.
- Step 4: first thermal reading at the chimney breast is 96 degrees, a 28-degree gap over the 68-degree ambient, well past the 10-degree trigger. Fail, stop rule taken. A recheck five minutes later reads 95 degrees, essentially flat. The gap has not measurably narrowed, so the crew treats it as still active, calls it in, and holds the perimeter until the fire department confirms it is safe to approach.
- Step 5: once cleared by the responding crew, the appliance is tagged out of service on site; a camera pass confirms no visible glow before the written notice is signed by the homeowner.
- Step 6: photos taken of blistered paint near the damper and a section of glazed, popcorn-textured creosote near the smoke shelf, both logged by location before any further cleaning.
- Step 7: Level 2 video scan scheduled for the following week; the cleanout door opened for the camera pass is reseated and confirmed to latch fully before the crew leaves.
The step 4 failure is the part worth keeping. The visible cues alone would have read as ambiguous to a less careful crew, and the thermal recheck is what turned an uncertain "seems okay now" into a confirmed still-active call, which is the exact judgment this procedure exists to remove from guesswork.
When the response does not go as written
A solo technician finds the cues with no second crew member on site. The headcount step collapses to one person; make the evacuation and 911 calls first, from a position clear of the structure, before doing anything else. Do not attempt the thermal recheck alone from a ladder if any doubt about active fire remains.
The fire department is dispatched but declines to treat it as an active event on arrival. Their assessment of what is currently active is authoritative for the emergency response; the lockout, escalation, and documentation steps in this procedure still apply regardless of that call, since a "no active fire" finding does not clear the chimney for use.
The customer is mid-use when the crew arrives and the cues are noticed before work even starts. Steps 1 through 4 still apply in the same order; the difference is the customer, not the crew, needs the first evacuation instruction, and that instruction takes priority over introducing yourself or reviewing the work order.
