Ladder and Roof Equipment Daily Inspection Standard

Purpose

A ladder, roof ladder, or harness fails at the worst possible moment to discover it: with a tech's full weight on it, at the chimney, above the eave. For this trade that is not a rare occurrence, it is most residential visits, since top-down access is the default sweep method. Almost every failure that reaches that point was visible on the ground first, in a cracked rail, a hook that no longer seats flush, or webbing worn thin at a buckle, and almost every one got skipped because the crew was already running late for the first stop. This standard is the three-to-five-minute check that happens before that, on every piece of access equipment, every morning it goes on the truck.

Scope

Covers the daily, pre-first-use inspection of the extension ladder, the roof ladder (ridge hook), the harness and restraint or arrest lanyard, and the anchor-plate hardware carried for chimney work. It does not cover the site-specific rigging, anchor rating, and work-at-height procedure performed at a given roof, owned by the Roof Access and Fall Protection Standard for Chimney Work, or the brush and rod tool string, owned by the Brush and Rod Equipment Inspection Standard. This standard is the go or no-go check performed before any of this equipment leaves the shop or is set against a roof.

The procedure

  1. Run a hand along both rails of the extension ladder, full length, checking for cracks, dents, bends, or corrosion. On fiberglass, the standard choice for this trade given proximity to service masts and overhead lines at many chimney locations, feel for delamination (exposed white fiber or a spider-crack pattern under the gel coat). Acceptance: no crack, delamination, or bend deeper than a light surface scratch, confirmed by touch, not a glance. Wrong looks like a rail that reads fine standing up but has a hairline crack near a rung socket only a hand would catch. Stop rule: any crack, delamination, or bend removes the ladder from service immediately; there is no "still fine, just watch it" tier for a compromised rail. Hazard: a cracked rail fails without warning under load, typically mid-span with a person's weight near the top, which is exactly why this check happens on the ground first.

  2. Grip and twist every rung by hand, and test both feet through their full pivot range independently. Check rungs for play, cracking at the rung-to-rail joint, and a slip-resistant surface not worn smooth or coated in dried sealant from the last job; check feet for full pivot, intact pads, and no packed debris jamming the swivel. Acceptance: every rung fixed with no play or cracking, both feet pivot fully with pads intact. Wrong looks like slight rotational play at one rung end, which is the joint starting to fail, not a cosmetic issue, or a foot that grips unevenly and reads as an unexplained wobble once standing, blamed on the ground instead of the equipment. Stop rule: any rung with play or cracking, or any foot that binds or is missing material, pulls the ladder from service; a shim or a folded rag is not a field fix. Hazard: a rung that lets go under foot load drops the foothold instantly with no warning, and a compromised foot is the single most common contributor to a base kickout, diagnosable only before weight is on the ladder.

  3. Extend the ladder fully, confirm both rung locks engage the same rung simultaneously, and test the extension with a firm downward pull. Acceptance: both locks seated on the same rung, the extended section holds firm under the pull test with no creep, rope or cable shows no fraying. Wrong looks like one lock catching a rung early because the rope stretched unevenly; the ladder looks extended and locked, but one rail is actually unsupported at that rung and racks sideways the moment weight loads it. Stop rule: any lock mismatch or creep under the test removes the ladder from service; do not hand-true the extension and call it fixed. Hazard: a mismatched or slipping lock lets the extension shift or collapse mid-climb, a fall from whatever height the ladder had reached.

  4. Read the duty-rating stamp and confirm it covers today's actual load: body weight, tool belt, and the rod and brush string. A sweep carries more up the ladder than a typical service call, a coiled rod string and brush add real weight to the tool-belt figure. Acceptance: rating stamp legible and equal to or above the combined load. Wrong looks like assuming "it's the shop's ladder, it's fine" without reading the stamp and adding the rod string's weight into the total. Stop rule: an illegible or missing rating stamp means the ladder is treated as unrated and pulled from service until confirmed against manufacturer records. Hazard: none new, a documentation and capacity check on the ground.

  5. Inspect the roof ladder's hook for weld integrity and wear, and confirm its wheels turn freely. Check the hook's welded joints for any crack or repair weld, the hook's bearing surface for wear that would keep it from seating flush over a ridge, and that both wheels spin without binding, since a wheel that drags makes the roof ladder harder to control while it is being run up an occupied slope. Acceptance: no weld crack, hook surface shows no wear deep enough to change its seating angle, both wheels spin free. Wrong looks like a hook with a visible old repair weld that "has held so far." Stop rule: any weld crack or repair, or a hook surface worn enough to change how it seats, retires the roof ladder; a dragging wheel gets cleared of debris or replaced before use. Hazard: a hook that fails at the ridge drops the roof ladder, and anyone on it, without the warning a rail crack gives on a standard ladder, since the failure point is a single small weld rather than a full-length rail.

  6. Inspect the harness webbing and hardware, the lanyard, and count the anchor-plate hardware against the truck's stocked inventory. Check webbing for cuts, UV fading, or fraying at stitch points, confirm the D-ring and buckles show no cracking, and flex the lanyard checking for a deployed shock-pack indicator or stiff, sun-damaged webbing. Count anchor plates and their matched fasteners against the stock list, since a plate with the wrong fastener set for today's roofing material is as good as no plate. Acceptance: no cut, fray, or UV damage on webbing, hardware shows no cracking, shock-pack indicator undeployed, anchor-plate count matches stock list with fasteners present. Wrong looks like webbing that still holds a tug test but shows visible fiber fuzzing at a stitch line. Stop rule: any webbing damage, hardware cracking, or a deployed shock pack retires that harness or lanyard on the spot; a missing anchor plate or fastener set gets restocked before that truck leaves. Hazard: a harness or lanyard defect found on the ground is inert; the same defect found at the ridge is a fall with no backup system, which is the entire reason this check runs before the truck loads, not at the roof.

  7. Log the result: pass, or condemned with the specific defect, naming the asset tag for each item checked. Acceptance: an entry made for the ladder, roof ladder, harness, and lanyard before any of them is loaded onto the truck. Wrong looks like a verbal "gear's good" to the crew with nothing written down. Stop rule: any condemned item gets a physical tag attached to it, not just a log entry, so a later shift does not pull it back into use without reading the log first.

The record this produces

The daily log ties a pass or fail to a specific asset tag and date, across ladder, roof ladder, harness, and lanyard. A lead scanning a season of entries watches for the same asset tag condemning repeatedly, which points at a specific unit's age or a specific crew's handling, not at the equipment class generally. A tag-out with no matching replacement noted that day's job log is a gap worth asking about directly.

Worked pass, with a step that failed

Monday morning, Ladder 6 (28-foot Type IA fiberglass extension), Roof Ladder 2, and Harness 4 with its 6-foot restraint lanyard. Step 1: both rails checked full length, no cracks or delamination. Step 2: every rung gripped and twisted clean, both feet pivot fully with pads intact. Step 3: extended fully, both locks seated on the same rung, downward pull test holds with no creep.

Step 4: rating stamp reads Type IA, and today's load (body weight, tool belt, and the rod and brush string) sits comfortably under it.

Step 5 FAILED and took its stop rule. Roof Ladder 2's hook shows a visible repair weld along one side, added sometime in the past season, with a hairline crack starting to reopen at the repair's edge. The stop rule fires: this roof ladder is retired on the spot, tagged "hook weld crack, do not use," and Roof Ladder 5 is pulled from the rack as the substitute. Roof Ladder 5 is inspected fresh against this same step: no weld damage, both wheels spin free. That fresh check on the substitute unit is the resume, not a note that "the spare is probably fine."

Step 6: Harness 4's webbing and hardware check clean, the lanyard's shock-pack indicator is undeployed, and the truck's anchor-plate count matches its stock list with fasteners present. Step 7: the log records Roof Ladder 2 condemned and referred to the shop, Roof Ladder 5 logged as today's substitute with a clean pass, and Ladder 6 and Harness 4 both logged clean.
