Underground Utility Strike During Footing Dig Response
Purpose
The auger has already hit something, or the crew cannot be certain it has not. Every minute spent deciding what to do next is a minute inside whatever hazard the contact created, and a deck crew's habitual response, stop and look, is exactly wrong for a gas or electric contact. This procedure exists so the first sixty seconds are identical no matter who is on the auger, and so the aftermath produces a written record instead of a story two people remember differently. The sibling locate SOP prevents most strikes; this one governs the ones that happen anyway, because a mark was wrong, a private line was never located, or the tolerance measurement was off by the inches that mattered.
Scope
Covers the moment of contact or suspected contact through reauthorization to resume excavation: hazard response by utility type, the mandatory damage report, exclusion zone and reentry, redesign of the hole, and the closing record. Applies whenever excavation contacts a buried facility hard enough to move, cut, dent, or crush it, or the crew cannot tell whether it has.
Does not cover the locate itself or exposure of an intact, undamaged facility, which deckbuilding-underground-utility-locate-before-digging owns start to finish. Does not cover the footing's depth, diameter, or its own inspection once digging resumes; deckbuilding-footing-layout-and-inspection-readiness picks the job back up there. Does not set your state's damage-reporting window itself; step 1 of the locate SOP already put that value on the job template, and this procedure reads it.

Procedure
1. Kill the machine and clear the ground, before anyone looks. On any contact, or any uncertainty, cut power at the machine, not just release the control, and everyone moves to a rally point at least 50 ft away and upwind, crossing the street if the layout allows it, before anyone assesses what was hit. Acceptance: every crew member accounted for at the rally point. Wrong looks like one person staying at the hole to see how bad it is. Stop rule: any doubt about what was struck is treated as the worst case, a gas contact, until proven otherwise. Hazard: an idling machine near a damaged line is itself an ignition source and a step-potential hazard, so killing it is first, not second.
2. Treat any gas odor, hiss, or bubbling as a gas contact. Everyone leaves on foot, moving upwind and clear of low ground where gas pools; nobody starts an engine, uses a phone, keys a radio, or flips a light switch inside the zone or any downwind structure; nobody plugs or crimps the line. Acceptance: the area silent of engines and radios, and 911 and the gas utility both called from outside the zone. Wrong looks like a call made from the truck at the curb, still inside the plume's path. Stop rule: the job stops for the day; nobody backfills a struck gas line. Hazard: gas can be heavier or lighter than air by blend, so do not assume it disperses upward, and treat any low or enclosed space near the leak as unsafe.
3. On an electric contact, keep the operator on the machine if energized and keep everyone else back beyond any arcing or ground disturbance. If the operator must dismount, they jump clear feet together and shuffle away in small steps, never bridging two points of different ground potential. Acceptance: the operator stays mounted until the utility confirms de-energized, or has executed the jump-and-shuffle exit. Wrong looks like a normal ladder-step dismount, which puts one foot at a different potential than the other. Stop rule: nobody re-approaches the machine or nearby ground until the utility confirms de-energized and grounded. Hazard: energized soil around a struck conductor can carry a lethal gradient well beyond where damage is visible.
4. On a water, sewer, or communications line, shut the excavation down and notify the owner. Most of these carry no immediate life-safety hazard, but a fiber trunk or sewer main carries commercial and public-health consequences that scale fast. Acceptance: the owner called, with a reference number. Wrong looks like patching a cut sewer lateral with tape and continuing. Stop rule: excavation in that hole stops until the owner assesses the damage, even with nothing actively leaking. Hazard: a damaged sewer or septic line can release pathogens into the open hole, so treat standing water or odor there as contaminated.
5. File the mandatory damage report the way the locate ticket was filed, on the record. Nearly every state's damage prevention statute requires reporting any contact to the facility owner and the one-call center, inside the window already written on the job template under the locate SOP's step 1. Acceptance: the report filed inside that window, with a reference number, regardless of whether the facility appears damaged. Wrong looks like reporting only strikes that obviously caused harm. Stop rule: uncertainty about whether contact counts as reportable defaults to reporting it. Hazard: none in filing; an unreported strike found later costs the shop its standing with the utility and the one-call center on every future ticket.
6. Hold the exclusion zone until the utility's own crew clears it, not until the hazard seems to have settled. Flag the zone at the radius the responding utility sets, and post one person to keep bystanders, the homeowner, and returning crew out. Acceptance: a named person posted from contact until reentry is authorized. Wrong looks like the flagging going up and the crew quietly resuming other tasks nearby. Stop rule: nobody crosses the boundary on their own judgment that it looks fine. Hazard: gas can persist or re-accumulate in low spots after the visible signs fade, so a quiet hole is not a cleared hole.
7. Get reentry authorization from a named person, not a feeling. Record who from the utility or emergency response cleared the site, their agency, and the time. Acceptance: a name, an agency, and a time before the zone comes down. Wrong looks like the responding truck leaving and the crew treating that as the all-clear. Stop rule: no name and time means the zone stays up until someone calls the utility back and gets one. Hazard: none once satisfied; the hazard was in skipping it.
8. Photograph and measure before anything else disturbs the site. Once reentry is authorized, document the facility's type, depth, and condition, the hole's position against the marks, and any spoil already displaced, before further disturbance. Acceptance: photos keyed to the footing mark, a measured depth to contact, and the ticket number cross-referenced. Wrong looks like photos taken after the crew has already widened the hole to see it better. Stop rule: if the owner is sending a crew, nothing is disturbed until they arrive or release it. Hazard: none in photographing; do not use the exposed line as a step or a brace.
9. Move the hole, do not push through it, and clear the new position against what was actually measured, not the original mark. If the footing's original position sits on or against a facility, the design changes rather than the excavation forcing past it, the same rule the locate SOP sets for an intact line. Acceptance: a revised footing position from the designer, with any post spacing change reconfirmed against the beam span table, and the new hole confirmed clear of the facility's step-8 measured depth and location by at least the locate SOP's standard tolerance, before it opens. Wrong looks like widening the original hole without rechecking the beam, or relocating a short distance while still trusting the mark that was just proven wrong instead of the depth and position actually measured at the strike. Stop rule: a spacing change goes back through the span tables first, and no new hole opens until its clearance is confirmed against the step-8 measurement, not the original mark; this check runs whether or not the new position falls inside the original ticket. Hazard: a mark just proven inaccurate cannot be trusted for the new hole's clearance any more than the first one; the new hole returns the job to the locate SOP's full tolerance-zone and re-mark rules regardless of the ticket boundary.
10. Close the incident into the job record before resuming excavation anywhere on the property. Acceptance: the incident timeline, damage report reference, reentry authorization, and any redesign all filed before the crew digs another hole. Wrong looks like moving to the next footing while the report is still getting written up later. Stop rule: excavation elsewhere does not resume until this record is complete. Hazard: none remaining, which is the point of closing the loop.
The record this produces
One Utility Strike Incident Record per event: date, time, and crew present; which facility was struck and by what evidence; actions taken under steps 2 through 4; the damage report reference and its window; the exclusion zone boundary and who staffed it; reentry authorization name, agency, and time; photos and measured depth; the redesigned footing position and any span reconfirmation; and the resumption date.
The office reads it if the facility owner or the homeowner's insurer disputes the damage. The estimator reads it on the next job at that address, since a private line never located once tends to still be unlocated. The next crew reads the redesigned position so nobody re-digs the original hole out of habit.
One run, including the stop that fired
Six footings on a ticketed, marked, privately located site; footing 3 sits inside the tolerance zone of a marked gas service and is being hand dug.
At about 22 inches the crew felt resistance and a faint hiss started. The lead called it a gas contact on sound alone and gave the kill-and-clear order. Step 1 fired clean: the auger was already off, and the two crew at the hole moved to the truck at the curb, joined seconds later by the rest of the crew.
Step 2 is where the stop landed. One crew member, out of habit, pulled a phone to call the lead, who was 40 ft away and visible. The lead caught it before the call connected; the rally point sat inside what should have been the exclusion zone, close enough that a spark risk was real. The crew relocated another 60 ft upwind and across the street, and 911 and the gas utility's emergency line were called from there.
The utility confirmed a small service line nick rather than a rupture, capped and repaired it, and gave reentry authorization to the lead by name about ninety minutes after contact. Once reentry opened, step 8 measured the actual contact point at 22 inches deep and about 6 inches off the original mark's line. Step 5's report was filed by the office within the window already on the job template. Step 9's redesign moved footing 3 fourteen inches toward the house, confirmed clear of the ledger setback and unchanged in post spacing, and confirmed against the step-8 measurement, not the mark, that the new position sat outside the locate SOP's tolerance zone from the line's real location. Excavation resumed the next morning once the redesigned position was re-marked.
What the pass shows: the stop rule that mattered was not about the gas line, it was about how clear the area really needs to be, and it fired on a habit rather than a conscious decision.
When the site does not match this procedure
If the strike happens outside the ticketed and marked area entirely, treat it the same for the immediate response, but the office's report and any conversation with the utility should say so plainly; an unmarked, unticketed hole is a different liability posture than a marked one that was wrong. If the facility owner cannot respond same day, the exclusion zone and posted watch stay in place overnight, and the crew works elsewhere on the property until the redesign clears.
