Confined Space Entry Verification Before Tank Work Standard
Purpose
This SOP produces a documented, on-site classification of a septic structure as confined space, permit-required or not, and entry or non-entry, made by the lead tech at the opening before the gas-testing SOP's first step runs, so the crew and equipment on site match what the structure actually requires rather than what a phone call assumed.
Without it, the gas-testing SOP has nothing to key off of. Its own first step, confirming the job is non-entry, is a dispatch-desk flag, not a verification made standing at the opening, and a site that changed since that call, an added extension, a stuck component turning a pump-out into an entry job, a wet well nobody has visited in years, will not correct itself. The recurring failure is a crew that proceeds on the non-entry classification because the ticket says so, discovers mid-job the work needs reaching past the opening plane, and improvises an entry with none of the permit, attendant, or rescue provisions a properly classified entry would have had.
Scope
Covers the classification decision for any onsite structure with a removable cover, before atmospheric testing or entry begins: septic tanks, pump and dosing chambers, aerobic treatment unit compartments, grease interceptors, lift station wet wells, and distribution boxes deep enough to hold a headspace.
Does NOT cover the atmospheric test itself, owned by the gas-testing SOP, which runs immediately after this SOP hands off a non-entry classification. Does NOT cover the mechanics of a permit-required entry once authorized, permit issuance, attendant duties, retrieval operation, owned by the shop's full 29 CFR 1910.146 entry program; this SOP decides whether that program is triggered, it does not run it. Does not cover excavating to a buried structure, owned by the tank locating and lid excavation SOP, or riser repair technique, owned by the access restoration how-to; both hand off here the moment a cover is exposed. In an OSHA State Plan state, confirm your state's confined-space standard, at least as protective as the federal rule.

Procedure
1. Check every structure on the ticket against the confined-space definition before the truck leaves. At booking, read the structure type off the record, tank, pump chamber, ATU compartment, distribution box, lift station, against the three-part test at 29 CFR 1910.146(b): large enough for a person to bodily enter, limited or restricted means of entry or exit, not designed for continuous human occupancy. Acceptance: every structure on the ticket marked confined space or not, in writing, before dispatch. Wrong is a ticket that just says "pump the tank," classification left to be inferred on arrival. A shallow distribution box a person cannot fit through does not meet the definition; almost everything else on a septic route does, and treating the exception as the rule is how the one structure that needed the check skips it.
2. Re-verify the physical classification from outside the structure on arrival, before the cover is touched. Confirm riser diameter and height, egress features visible without opening it, a fixed ladder or rungs seen through a grate or in prior photos, and depth to liquid from the last pump record, against what dispatch assumed. Acceptance: the step 1 classification confirmed or revised in writing before the gas-testing SOP's first step runs. Wrong is proceeding straight into gas testing on the phone flag alone; the stop rule is reclassify first, because a buried riser that has been extended, or a wet well nobody has serviced since the record was written, will not match a classification made over the phone.
3. Screen for permit-required hazards beyond atmosphere. Check engulfment potential from liquid depth or volume, entrapment by converging walls, an internal baffle, or a sloped floor, and any other recognized serious hazard, an energized submersible pump, a running auger or mixer, against what is visible or documented for that structure. Where a pump or mixer is present, lock out its circuit at the panel and confirm de-energized with a meter at the equipment itself before anyone treats it as isolated; a breaker labeled off is not the same as a motor proven dead. Acceptance: each hazard category marked yes or no with the physical basis noted, not left blank, and any pump or mixer's isolation confirmed by meter, not by breaker position alone. Wrong is screening only for gas because that is what the next SOP tests for; a wet well with an energized grinder pump is permit-required on the mechanical-hazard clause alone even where atmosphere and engulfment both pass, and the pump's isolation status gets verified here, not assumed clear because it usually is.
4. Reconcile the on-site classification against the ticket's phone-flagged designation. Compare what step 2 and step 3 found to what dispatch wrote at booking. Acceptance: they agree, or a documented reclassification, called in to the service manager, replaces the phone flag before anything else proceeds. Wrong is quietly working around a mismatch because the crew is already on site and the job looks small; the stop rule is that a reclassification is not optional paperwork, it changes which crew, paperwork, and equipment the rest of this SOP requires present.
5. Verify crew, paperwork, and rescue equipment on site actually match the classification just confirmed. An entry classification requires an entry permit, a named entry supervisor, an attendant whose only duty is attending, and a rigged retrieval line, with a mechanical retrieval device for a vertical drop over 5 feet per 29 CFR 1910.146(k)(3) unless a device would increase the risk. Acceptance: each item present and named on the ticket for the classification in play. Wrong is an entry-classified job worked with the non-entry gas-testing kit and a two-person crew where the second person also runs the vac truck; the stop rule is secure the space, do not enter, and call for the missing crew or gear.
6. Confirm the attendant can summon rescue without leaving the station. Test cell signal, or a second working means, at the actual opening, not assumed from what the truck showed in the driveway. Acceptance: a working communication method confirmed at the opening itself. Wrong is assuming coverage because the phone had bars on the way in; the stop rule is do not open the cover or authorize entry until confirmed at the opening, because an attendant who has to walk off to find signal has just removed the one thing 29 CFR 1910.146(i) puts them there to provide.
7. Hand off the classification explicitly, naming which path governs the rest of the visit. Annotate the ticket with either non-entry, proceed to the gas-testing SOP, or entry, permit issued under the shop's 1910.146 program. Acceptance: the path is named in writing on the ticket, not left for the next step to infer. Wrong is leaving the classification implied by whatever happens next; a reviewer reading the ticket later has to read the call, not reconstruct it.
8. Reclassify from step 1 the moment the job's physical requirements change mid-visit. A stuck component that requires reaching past the opening plane, liquid deeper than the record showed, or a hazard not previously screened, each voids the original classification. Acceptance: work stops, the opening is secured, lid restored or continuous monitoring maintained, while steps 2 through 7 run again in full before anything resumes past what the original classification authorized. Wrong is treating a mid-job scope change as a small extension because the crew is already most of the way through it; the resume point is where a crew that classified correctly the first time improvises the second, so the stop rule holds regardless of how close to finished the job looks. Hazard: the physical change that triggers this step, a stuck component reached into, liquid deeper than recorded, or a hazard not previously screened, is itself evidence the space no longer matches what steps 2 through 7 confirmed; the crew is exposed to an unverified engulfment, entrapment, or atmospheric hazard from the moment the change is noticed, so everyone exits to the surface for the re-run rather than working from inside the space while it happens.
When the site does not match
A structure with no prior record and no egress feature visible from outside defaults to permit-required until proven otherwise; the conservative classification is the safe guess, not a coin flip. A cover already off or broken on arrival, meaning someone may have opened it before the call came in, gets treated as an active, undefined opening rather than resumed under the ticket's classification; restart at step 2. A structure the lead tech genuinely cannot classify from what is available gets called in to the service manager before any tool touches the cover, not decided alone in the yard.
The record this produces
One classification record on the work order: structure type and identifier, the three confined-space criteria, the hazard screen for engulfment, entrapment, and other recognized hazards with its basis, the phone-flagged designation versus the on-site classification and any reclassification with who authorized it, the crew, permit, and rescue equipment confirmed present, the attendant's communication check, the path handed off, and, where step 8 fired, the mid-job trigger and full re-run.
The service manager reads reclassifications monthly to see where phone-desk assumptions are drifting from site reality. Whoever schedules the next visit reads the classification so the next ticket starts closer to correct. If an entry incident is ever reviewed, this record is what shows the classification was made, not assumed.
Worked pass: 1,500 gallon two-compartment tank, riser to grade, effluent filter service
Ticket phone-flagged non-entry, pump and inspect, two techs. Step 1 confirmed a septic tank meets all three confined-space criteria; step 2 on arrival matched the record, 20 inch riser, no fixed ladder, depth to liquid about 4 feet. Step 3 hazard screen: engulfment potential from liquid depth, no converging walls or mechanical hazard on a passive gravity tank. Step 4 reconciled clean against the non-entry flag, step 5 confirmed the non-entry kit and two techs, step 6 confirmed cell signal at the opening, step 7 handed off to the gas-testing SOP, which passed.
Mid-pump, the effluent filter housing was found jammed by a root mass the standard filter tool could not reach from outside the riser. Step 8 fired. Freeing it required a hand and forearm past the opening plane, which is entry. Work stopped, the cover was restored to secure the opening, and steps 2 through 7 ran again against an entry classification: 4 feet sits under the 5-foot mechanical-retrieval trigger at 1910.146(k)(3), but the permit, a named entry supervisor, and a dedicated attendant were still required, and none were on this ticket, built for a non-entry visit. Step 5 failed on the second run: the second tech was also running the vac truck, not free to attend exclusively, and no permit paperwork was on board.
Under the step 5 stop rule the crew did not improvise the entry. The opening was secured, the service manager was called, and the job was rescheduled with a full entry crew, permit paperwork, and a dedicated attendant.
Checking the run against the rules above: the reclassification at step 8 reused the same criteria and hazard screen as steps 1 through 3 rather than a shortcut version, and the site was left secured, cover restored, exactly as step 8's acceptance condition requires.
